PCN Manager

Job summary

Bradford North West PCN are looking for a Primary Care Network Manager to project manage and support our PCN in its day to day running and in new developments and in promoting and nurturing excellence.

The post holder will form an integral part of the PCN leadership working alongside the Clinical Director, the clinical board, and the management teams of the constituent practices. This role is essential to the smooth running and progression of our PCN, and we are looking for a confident, dynamic individual with excellent project management, negotiation and communication skills.

Main duties of the job

• Support the PCN Clinical Directors and board members to ensure the smooth running of the network
• To attend PCN board meetings, managers meetings, Clinical Directors meetings and represent the PCN alongside the Clinical Director.
• Provide administrative support to the PCN: Co-ordinate meetings, set agendas, keep minutes, manage action logs
• Project manage quality improvement projects and support staff at all levels in the PCN to take forward change
• Use data to identify opportunities and to drive and monitor improvements in care quality and experience
• To present progress reports of activities across the PCN to the PCN board and to produce and deliver reports as required by commissioners.
• To coordinate the successful achievement of all PCN DES contractual requirements, support the translation of national and local policies into meaningful policies, and support collaborative working between multiple organisations to achieve improved outcomes for the local population
• Management and monitoring of PCN finances and budgets working with PCN leadership team to ensure fair distribution and reconciliation of financial funds and available DES funding
• Champion the induction and the use of existing and new technology to deliver benefits to patients and staff
• Develop and facilitate a PCN wide patient participation group. This to include PCN questionnaires, surveys and analysis and presentation of results
• Support individual Practices understand and deliver PCN objectives and requirements
