Job summary

Jesmond Health Partnership (JHP) is a forward-thinking GP practice operating across four sites in the Jesmond and Gosforth areas. We are seeking a dedicated and compassionate Health Care Assistant (HCA) to support the delivery of high-quality care to our patients as part of our established multidisciplinary team.

The HCA will assist with a range of clinical and administrative tasks under the supervision of registered healthcare professionals. Duties typically include phlebotomy, blood pressure checks, Health Checks, ECGs, vaccinations, wound care, and assisting with chronic disease management. The role also involves supporting infection control, maintaining equipment, and accurately recording information on clinical systems.

This is a dynamic role that may require working across all four JHP locations, offering the opportunity to contribute to patient care in a flexible and supportive environment committed to continuous improvement and professional development.

Main duties of the job

The Health Care Assistant will be required to promote and support the mission and vision of the service for which they are responsible. They will:

Ensure strict confidentiality.

Adapt to and support any changes that are implemented to improve the service.

At all times provide a caring service and to treat those with whom they come into contact in a courteous and respectful manner.

Demonstrate their commitment by the efficient completion of all tasks allocated to them.

Carry out their duties and responsibilities in compliance with local policy and statutory regulations.

Phlebotomy

Ear syringing

Wound management / suture, clip removal

Administer B12 injections, flu and shingles vaccines as per practice guidelines, with appropriate training

Hypertension reviews.

Diabetic foot checks

Health Checks

Referring appropriately to GPs and to the wider clinical team as necessary.

Treatment Room Preparation & Maintenance

Preparing and maintaining environments and equipment before, during and after patient care interventions.

Clearing and re-stocking consulting rooms

Equipment and vaccine re-stocking and stock rotation

Help ensure availability of appropriate stock by following stock control procedures

Sample & Vaccine Management

Processing and management of laboratory samples requested

Vaccine/cold chain storage, monitoring and recording

Support During Clinical Interventions

Chaperoning duties.

Assisting in the assessment and surveillance of patients health and well-being

Undertaking specific clinical activities for named patients that have been delegated and taught specifically in relation to that individual

Supporting patients who are anxious or stressed

Perform duties with sensitivity to individual patient needs

Health Promotion & Patient Education

The promotion of health and well-being in the practice population.

Assisting with the collection and collation of data on needs related to health and well-being particularly QOF.

Administrative & Record Keeping

Use of recall system

Prioritise, organise and manage own workload and recognise and work independently within own levels of competency

Take responsibility for own personal and professional development.

Be an advocate for both the patient and for your colleagues
