Job summary

The new role will operate in a dynamic clinical environment, responsible for providing best practice healthcare procedures for patients in our 5 GP surgeries and patients’ own homes. Working in partnership with a Registered Nurse, a Nursing Associate will deliver a safe, high standard of nursing care that promotes health and prevents ill health for our patient population.

You will work with patients at all 5 GP practice in North Gosforth, in collaboration with GPs, Advance Nurse Practitioners, Practice Nurses, District Nurses and community services.

We are looking for a registered Nursing Associate who has completed their Foundation degree.

This is a new role, with the opportunity to shape and develop the role of a Nursing Associate within a multi-disciplinary team as well as to develop your own clinical skills. Problem solving, communication skills and the ability to form effective working relationships will be essential to succeeding a dynamic environment.

You will be expected to work effectively as a part of our wider team, but also be able to independently manage and prioritise your own workload. Prior experience in a Primary Care setting – GP practice, urgent care, or walk-in centre – would be desirable.

Main duties of the job

The new role will operate in a dynamic clinical environment, responsible for providing best practice healthcare procedures for patients in our 5 GP surgeries and patients’ own homes.

You will work as part of our multi-disciplinary team to carry out pro-active management of the health of housebound patients including long term condition reviews, in line with the principle of the Year of Care.

Other typical duties will include physical observations, venepuncture, ECG recording, injections, and other procedures in line with your training. North Gosforth Primary Care Network is committed to the training and development of team members, and you will be supported to further your career in line with your personal and professional goals.

As a valued member of the team, you will contribute to the breadth of knowledge across the fields of nursing, providing holistic and person-centred care and support for people of all ages. This role will assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. In addition, the Nursing Associate will be expected to support the administrative team on an as required basis.

About us

PCNs were introduced into the NHS in 2019.  Essentially, it is a group of GP practices working together. They share resources and specialist staff, meaning each practice and their patients benefit from additional services that individual practices couldn't provide independently.

About Our Network:

North Gosforth Primary Care Network is made up of 5 neighbouring GP practices: Broadway Medical Centre, Brunton Park Health Centre, Gosforth Memorial, Park Medical Centre, Regent Medical Centre

North Gosforth PCN works collaboratively with Jesmond Lower Gosforth PCN, meaning that specialists from each network work together, sharing best practices and raising standards, and teams can collectively provide the best holistic care for patients.

You will be a registered Nursing Associate, and will have completed your Foundation Degree. You will work as part of a Primary Care Network multidisciplinary team, providing safe, person centered care to improve the health of our population.

In line with best practice and with support from the MDT, you will carry out various procedures to monitor and improve the health of patients, including physical observation, venepuncture, ECG recording, injections, and other procedures commensurate with your training and skills. In particular, you will undertake proactive monitoring and other procedures for housebound patients to enable Long Term Conditions reviews and management in accordance with the principles of the Year of Care (YOC).

You will work in collaboration with a range of other professionals including GPs, ANPs, practice nurses, district nurses, and other community services.

Working arrangements:

Monitor the condition and health needs of patients within their care on a continual basis in partnership with patients, families and carers, ensuring appropriate escalation to a registered nurse or other professional when required.

Promote comfort and well-being by ensuring that patients’ personal and social needs are met and be able to care for a patient’s nursing needs.

To safely undertake the following procedures to meet the patient’s nursing needs to

include; undertaking routine procedures including venepuncture, ECG recording,

measuring and interpreting blood glucose levels, recording and interpreting vital signs, continence care, bladder and bowel care, pressure area care, wound care, nutrition, hydration, foot care, skin integrity, mood identification (as outlined in the NMC Nursing Associate Skills Annex). Identify and report changes in a patient’s condition to a registered nurse or other professional.

Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members.

Provide and receive sensitive information concerning a patient’s medical condition.

Work with registered nurses and other professionals and engage with other agencies and community teams to ensure a safe, individualised approach to the admission and discharge of patients.

Maintain accurate, contemporaneous patient records in line with [PCN/practice] policy and the NMC Code.

Provide effective evidence based care for service users under the supervision of registered nurses and other professionals in order to perform non-complex activities, in line with clinical governance and professional standards.

Communication:

● Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment

● Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
