Job summary

The clinical social prescriber link worker role, will deliver a safe, high standard of care that promotes health and prevents ill health for our patient population.

This new role will support the PCN and you will be required to carry out monitoring and healthcare improvement skills in line with year of care, annual reviews, and personalised care support planning and health improvement strategies. This will predominantly include patients in care homes and in the wider housebound community as part of our over 65 team. You will work in collaboration with a range of other professionals including GPs, ANPs, practice nurses, district nurses, and other community services.

You will be working as part of the Jesmond and Lower Gosforth PCN team to support the patients and practices of Jesmond and Lower Gosforth.

Main duties of the job

We are looking for a registered Nursing Associate who has completed their Foundation degree and holds professional registration with the NMC.

This is a new role, with the opportunity to shape and develop the role within a multi-disciplinary team as well as to develop your own skills.

Problem solving, communication skills and the ability to form effective working relationships will be essential to succeeding.

You will be expected to work effectively as a part of our wider team, but also be able to independently manage and prioritise your own workload.

Prior experience in a Primary Care setting GP practice, urgent care, or walk-in centre would be desirable.

About us

This is a new role working in NHS Primary Care Networks and GP practices.Primary Care Networks or PCNs were introduced into the NHS in 2019.

Essentially, it is a group of GP practices working together. They share resources and specialist staff, meaning each practice and their patients benefit from additional services that individual practices couldn't provide independently.

The number of practices in a PCN varies, but typically they cover between 30,000 and 50,000 patients.

Each network has a board that oversees how the PCN is run, with members coming from each of the GP practices and a Clinical Director who has overall responsibility for the services the network delivers.

Primary Care Networks are contracted to provide health and care services to patients through GP practices. Jesmond and Lower Gosforth Primary Care Network is made up of 4 neighbouring GP practices:

Roseworth Surgery (5,500 patients)

Clinical practice:

Providing and monitoring care based on evidence-based practice working in partnership with the multi-disciplinary team in a range of community settings.

Monitor the condition and health needs of patients in accordance with year of care and annual reviews ensuring escalation to appropriate professional colleagues / team when required.

Support patients to improve and maintain their mental, physical, behavioural health and well-being recognising health inequalities and proactively working to reach people from all communities.

To safely undertake the following procedures to meet the patients nursing needs to include; venepuncture, ECG recording, measuring and interpreting blood glucose levels, recording and interpreting vital signs, assessing continence care, bladder and bowel care, pressure area care, basic wound care, nutrition, hydration, foot care, skin integrity, mood identification assessments, within your scope of practice and identify and report any concerns to a registered nurse and/or refer to other Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carers and family members.

Provide and receive sensitive information concerning a patients medical condition.

Maintain accurate, contemporaneous patient records in line with [PCN/practice] policy and the NMC Code.

Adhere to local safeguarding policy.

Ensure incident reporting guidelines are adhered to including sharing of good practice/outcomes.appropriate professional.

Communication:

Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating including motivational interviewing.

Anticipate barriers to communication and take action to improve communication

Maintain effective communication within the PCN and the wider MDT.
