Job summary

Jesmond Health Partnership is looking to recruit an individual to work as part of our Acute Care Team. The individual may be an Advanced Nurse Practitioner, Emergency Care Practitioner or a Paramedic Practitioner, preferably with experience of working within Primary Care.

We wish to build on the wider skill mix we have developed within our team and look at how we can innovate and work differently, whilst developing a model that works for the benefit of our Patients and Staff. Our current Acute Care team handle the majority of urgent access on a daily basis as well as urgent visiting to housebound patients. We are looking to expand the Acute Care team to develop resilience in our provision of urgent access including the proactive management of the housebound / frail population. We are a digital first GP practice and are keen to support colleagues developing further skills to carry out their role in this new and innovative way of working.

Main duties of the job

We are looking for an enthusiastic colleague to help contribute to our vision for Primary Care and are willing to be flexible for the right candidate. The role is full time, available immediately and we would consider flexible working needs.

JHP currently comprises of:

A combined list of approximately 13,500 patients across two sites less than one mile apart.

Systmone on both sites.

High QoF Performance.

Good across all areas in last CQC.

Significantly better than local or national results for GP Choice feedback.

We would ask that prospective applicants have the following qualifications and skills

Confident to work autonomously and manage own clinic/case load within a supportive team

Completion of clinical skills course with recent and relevant experience

Independent prescribing (non-medical)

Experience in Primary care / Community care

Caring, sensible, hardworking personality

Good communication skills

About us

We are introducing formal and informal peer support across the sites including practice education sessions. We have a history of working innovatively and would welcome colleagues looking to develop their role further, with support, in the future.

Both sites are situated in Jesmond. The plan is to continue to run the larger practice from both sites.

  • Ensure accurate and legible notes of all consultations and treatments are recorded in the
  • patients notes.
  • Ensure the clinical computer system is kept up to date with accurate details recorded and
  • amended; this includes accurate use system1 read codes.
  • Ensure accurate completion of all necessary documentation associated with patient health
  • care and registration with the practice.
  • Ensure collection and maintenance of statistical information required for regular and ad hoc
  • reports and audit.
  • Attend and participate in practice meetings as required.
  • Provide assessment, screening treatment, services and health education advice.
  • Assist with the organisation and co-ordination of the provision of nursing services for the
  • practice.
  • Provide nursing treatment to patients in participation with general practitioners using
  • agreed protocols, this may include on occasion the taking of cervical smears.
  • Provide general and specific health screening to the practice patients (within the agreed
  • protocols) with referral to General Practitioners as necessary.
  • Be competent in administration of childhood immunisation and travel vaccination.
  • Be competent and confident in managing people with long term health conditions.
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients
  • presenting with an undifferentiated diagnosis.
