Job summary

This is a new role working in NHS Primary Care Networks and GP practices.

Primary Care Networks are contracted to provide health and care services to patients through GP practices. This role will help coordinate that work through business intelligence and IT systems support.

Liaising with GP practices and staff, monitoring contractual performance with robust processes and key metrics, with regular reviews and reporting to stakeholders are all central to this role.

Leading on information governance and the provision of effective and reliable IT systems will also be essential.

Identifying and developing new opportunities will also be vital in establishing and growing both this role and the work of the Primary Care networks.

Main duties of the job

We are looking for someone who can play an integral role in developing and delivering high-quality health and care services that genuinely benefit our patients' long-term health and well-being.

You will have an eye for detail and create and implement processes that colleagues can follow.

Comfortable working with a range of people across the NHS from practice teams, network staff, board members and stakeholders, you will also be able to confidently deputise for the Clinical Directors and Strategic Manager, as required.

A strong understanding of the NHS, especially the role, aim and challenges of primary care, is essential. Previous experience of working in a primary health care setting would be advantageous.

About us

Primary Care Networks or PCNs were introduced into the NHS in 2019.

Essentially, it is a group of GP practices working together. They share resources and specialist staff, meaning each practice and their patients benefit from additional services that individual practices couldn't provide independently.

The number of practices in a PCN varies, but typically they cover between 30,000 and 50,000 patients.

Each network has a board that oversees how the PCN is run, with members coming from each of the GP practices and a Clinical Director who has overall responsibility for the services the network delivers.

About Our Networks:

While our two networks Jesmond Lower Gosforth (JLG) and North Gosforth (NG) are their own entities, they work together to maximise effectiveness.

This collaborative approach means that specialists from each network can work together, sharing best practices and raising standards, and teams can collectively provide the best holistic care for patients.

The networks also share a Strategic Manager who oversees the day-to-day running of both PCNs.

The successful candidate will report to the Strategic Manager, with the Jesmond Health Partnership practice their direct employer.

Accountable to: Clinical Director

Reports to: Strategic Manager

Location: Various GP practices across Jesmond and Gosforth

Hours: 37.5 hours per week

Summary of Role;

To support the Strategic Manager by:

Day to day management of all data, business intelligence and IT systems and processes required by the PCN to ensure the effective and efficient management and delivery of contracted services

Day to day liaison with PCN practices, stakeholders and partner organisations to ensure the effective support of all service delivery undertaken and business development opportunities pursued by the PCN

Direct support to PCN practices, stakeholders and partner organisations as a service provided by the PCN

Deputising for the Strategic Manager, Clinical Director and PCN members in the event of their absence and or as required

Ensuring the effective review and management of PCN performance dashboards, contractual and stakeholder reports and returns

Day to day liaison with the PCN team

Ensuring robust contract performance and demand management systems and processes are developed and implemented

Being IG Lead and coordinating, publicising and monitoring the standards of information handling throughout the PCN, ensuring that employees are fully informed of their own responsibilities for maintaining the standards and that information about the standards is made available to the public where appropriate

Main Responsibilities of Role;

Support the Strategic Manager in the day to day running and development of PCN

Support the Strategic Manager and the PCN team in identifying and developing new business and service development opportunities

Support the PCN team and lead personnel in the day-to-day management of their functions and teams

Liaise with and support PCN practices to ensure the effective management and delivery of contracts and services undertaken by PCN

Identify areas for continual improvement within areas of responsibility and proactively assist with implementing change, as required

Undertake specific assigned tasks, project support and development work which may arise during the impending changes to the PCN and PCN practices

Proactive contribution to ensuring the operational efficiency of PCN and PCN practices

Report risks, significant problems and issues to the relevant person

Delivery of all PCN data and IT services according to specifications and requirements

Liaison with external partner organisations and stakeholders

Quality Management:

Ensure compliance with any internal or external audits, including CQC requirements

Ensure the development, maintenance and implementation of robust IT and data management systems and processes

Administration:

Ensure the administration of the data management and IT support to the PCN

Ensuring appropriate and timely data input as required

Document Management in accordance with PCN policies and procedures

Ensure urgent correspondence and items of relevance are immediately brought to the attention of the Clinical Director, Strategic Manager and PCN Board and respond to correspondence, as appropriate

Maintain up to date and efficient data and IT function office systems including filing
