Job summary
This is an exciting opportunity to work in an innovative
environment and complement our growing primary care team situated in East
London; a borough which has one of the highest prevalence of diabetes in the UK.
You will be working in conjunction with our Diabetes Specialist
Pharmacist, experienced HCAs, Nurses and Local Diabetic Nurse Specialists,
supported by our GP Lead in diabetes.
Main duties of the job
We are looking for an experienced Diabetes Nurse Specialist with strong clinical and leadership
skills who is passionate about providing high quality care and is able to work
autonomously.
The role involves:
managing Diabetes
care and its complications
act as an advisory
role to patients and doctors
to prevent
avoidable admissions to hospital
reduce
referrals into the traditional diabetes outpatient services

Pathways certificate in understanding the care and management of diabetes,
GLP1, SGLT2 training or equivalent training essential.
Overview of the role as a Diabetes Specialist Nurse, youll
offer the highest standard of care in a professional manner. Through your
strong clinical knowledge, you will provide expert knowledge, education, care
and support for all people with diabetes within the clinic including healthcare
professionals. You will also be involved in the management of complex caseload
of patients with diabetes to educate towards self-management, prevention and
early detection of cardiovascular risk factors and treatment of diabetes
complications. This includes patients with Type 1 and 2 Diabetes.
