Job summary
The role will involve structured medication reviews to reduce unnecessary polypharmacy's. To improve the monitoring of drug interaction, high risk medication as well as regular reviews of high risk drugs. We look after a few care homes and would like the candidate to become actively involved with the care home reviews.
Our PCN is very forward thinking, innovative and proactive
organisation made up of 3 closely working practices. We have a caring and
supportive learning environment. We are a training practice accredited to train
junior doctors and medical students. We are supporting our nurses and pharmacists
with post graduate advanced primary care diplomas. We are actively involved in
local leadership groups as directors of both CCGs and PCNs. We have a strong
stable team with a very low staff turnover. We would like the candidate to
become an active member of our multi professional teams.
Main duties of the job
We are looking for good clinicians with aspirations for
leadership roles that can show adaptability and enthusiasm to progress in their
career. For this role we require the following skills-:
Collaborate with patients, the public and other healthcare professionals to improve patient outcomes
Identify and employ the appropriate diagnostic and physiological testing techniques in order to promote health and to inform clinical decision making
Communicate with patients about their prescribed treatment
Establish and maintain patient relationships while identifying patients desired health outcomes and priorities
Obtain and record relevant patient medical, social and family history
Communicate information about available options in a way that promotes understanding
Support the patient in choosing an option by listening and responding to their concerns and respecting their decisions
Conclude consultations to ensure a satisfactory outcome Maintain accurate and comprehensive consultation records

The post holder will work as part of a multi-disciplinary
team in a patient-facing role. The post holder will take responsibility for
areas of chronic disease management within the practice and undertake clinical
medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general
practice staff with regards to prescription and medication queries. They will
help support the repeat prescription system, deal with acute prescription
requests, and medicines reconciliation on transfer of care and systems for
safer prescribing, providing expertise in clinical medicines advice while
addressing both public and social care needs of patient in the GP practice (s).

on medicines optimisation and quality improvement and manage some aspects of
the quality and outcomes framework and enhanced services. The post holder will
ensure that the practice integrates with community and hospital pharmacy to
help utilise skill mix, improve patient outcomes, ensure better access to
healthcare and help manage workload. The role is pivotal to improving the
quality of care and operational efficiencies so requires motivation and passion
to deliver excellent service within general practice.

- Mandatory registration with General Pharmaceutical Council
- Membership of the Royal Pharmaceutical Society
- Undergone CPPE Primary care pharmacy education pathway.
- Minimum of 2 years post qualification experience.

- Excellent interpersonal, influencing and negotiating skills
