Job summary
A Clinical Pharmacist plays a vital role in healthcare, working closely with physicians, nurses, and other healthcare professionals to ensure the safe and effective use of medications. Unlike traditional pharmacy roles that focus primarily on dispensing medications, clinical pharmacists take a more hands-on approach in patient care, optimising medication therapy, preventing adverse drug reactions, and improving patient outcomes.
Key Responsibilities:
- Collaborate with healthcare teams to develop and adjust medication plans based on patient needs.
- Conduct medication reviews and identify potential drug interactions or adverse effects.
- Educate patients and healthcare providers on proper medication use and management.
- Monitor patient responses to therapy and recommend adjustments as needed.
- Ensure compliance with regulatory standards and best practices in medication safety.
- Contribute to research, quality improvement initiatives, and policy development in medication management.
Essential Skills & Qualifications:
- A degree in Pharmacy (e.g., PharmD, MPharm, or BPharm) with clinical experience.
- Strong knowledge of pharmacology, therapeutics, and evidence-based medicine.
- Completion of the CPPE General Practice Pathway
- Excellent communication and problem-solving skills.
- Ability to work collaboratively in a multidisciplinary team.
- Experience with electronic health records and medication management systems.

PCN Clinical Pharmacist Maternity Cover (E4 PCN)
We are seeking an enthusiastic and adaptable Clinical Pharmacist to join our team for short-term maternity cover within a Practice in the E4 Primary Care Network (PCN). This role offers the opportunity to make a real difference in medicines management, patient safety, and quality improvement while supporting general practice.
About the Role
As a Clinical Pharmacist in Primary Care, you will play a key role in:
- Optimising medication use to ensure patients receive safe, effective treatments.
- Reducing hospital admissions by identifying and resolving medicine-related issues.
- Supporting general practice teams to improve prescribing safety and manage workloads.
- Providing clinical leadership on medicines safety and quality improvement projects.
- Considering public and social care needs in medication-related decision-making.

- Strong clinical knowledge and experience in medication reviews and prescribing.
- Proficiency in I.T. systems used within primary care.
- A proactive approach to medicines safety and quality improvement.
- Ability to work effectively within a multidisciplinary team.
- CPPE General Practice Pathway completed this is an essential requirement.

E4 Network is a Primary Care Network (PCN) consisting of five practices in Chingford, London, within the borough of Waltham Forest. We work collaboratively to ensure high-quality, efficient, and sustainable primary care services for our 54,000+ registered patients. Our mission is to support practices, patients, and staff by delivering innovative, patient-centred healthcare.
Our Clinical Pharmacist team consists of:
- Three Senior Clinical Pharmacists providing leadership, mentorship, and line management.
- Three Band 7 Clinical Pharmacists working closely with practices to optimise medicines management.
The team meets regularly with the Network Clinical Director, and Senior Pharmacists provide dedicated support and guidance to Band 7 Pharmacists during protected learning time. The pharmacists work as a network-wide team to improve prescribing safety and quality across all practices.
