Job summary

Folly Lane Medical Centre is looking for an enthusiastic and caring Mental Health Practitioner for21-30 hours per week to include either one early morning or evening clinic per week.

Some flexibility can be offered on days worked, although a Thursday is required.

The post holder will be a registered health care professional for example RMN or similar, an enhanced DBS reference will be required and they must have a driving licence.

Main duties of the job

Main duties of the role include-

Assessment of Mental Health presentations

Working with and signposting to relevant services such a talking therapies or other members of the Primary Care Team. Refer on to the Primary care mental health service and other parts of the secondary care mental health services as required.

Being a point of contact for those going through a Mental Health Crisis

Review of those recently diagnoses with Depression

Review of those on Long term Mental Health Medication

Proactive support to patients in mental health rehabilitation

Physical health assessments for those with Chronic Mental Health Conditions and ADHD

Dementia reviews

Completion of Safeguarding update reports

To formally and informally impart knowledge and skills to colleagues promoting peer review and best practice within the work environment.

To communicate at all levels within the team ensuring an effective service is delivered.

To maintain accurate, contemporaneous healthcare records appropriate to the consultation.

Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care.

Training and personal development

Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.

Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.

If it is necessary to expand the role to include additional responsibilities, full training will be given.

Develop and maintain a Personal Learning Plan

Liaison

As well as the nursing team, there is a need to work closely with reception, office to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team

There is also the need to establish and maintain good liaison with other surgeries and agencies, including secondary care
