Job summary

The post holder will be a qualified nurse with relevant clinical experience, ideally including previous work with frail, older people. The post holder will have knowledge of the common frailty syndromes with the relevant skills to be able to recognise and deal with the complexities that arise from these. The post holder will be supported by the Frailty ACPs as well as the PCN practices and management team.

The Frailty Nurse will provide patient centred care which will involve assessing and treating patients in their own homes. Giving advice and managing problems using a professional autonomous decision making processes, liaising when appropriate with other healthcare professionals and organisations to ensure safe and effective patient care.

They will provide appropriate patient care in accordance with practice protocols and policies. The post holder will actively engage patients and carers in management of their health, promoting health and wellbeing.

They will work collaboratively with the Integrated Care Community and primary care teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. Working within NMC code of conduct at all times.

The post holder will support the ACPs in recognising, negotiating and implementing change processes to enhance the delivery of care, both within the Practice and when completing home visits.

Main duties of the job

Main Responsibilities include

  • Clinical assessment and treatment of patients referred to the frailty team.
  • To attend the weekly MDT meetings where appropriate.
  • Care planning of appropriate patients and chronic disease management,including annual long-term conditions reviews.
  • Ensuring patient centred complex care planning, involving family and carers as appropriate.
  • Referral to appropriate services following clinical assessment.
  • Provide support to our frailty coordinators.
  • To help the PCN develop the frailty team and improve service delivery to produce an effective efficient service.
  • To help with training of team members where appropriate.

About us

Our Frailty Team is an innovative team created to visit and assess patients in their own home and care homes. It was created with the aim of providing clinical assessment at home for patients who may be acutely unwell or who may be at risk of hospital admission. Patients who have recently been discharged may also require clinical assessment by the team.

The PCN covers 39,000 patients registered with three Carlisle general practices. Our practices include :-

In North Cumbria we are developing integrated ways of working with our health and care partners including our North Cumbria Integrated Care (NCIC) Trust, Adult Social Care and voluntary sector organisations. Job responsibilities

The post holder will contribute to development and delivery of a high quality innovative service to the patients within the community, through the implementation of evidence based clinical practice, supported by continual professional development, and provide support and mentorship to all healthcare professionals.

Communication

1. Communicates effectively as a member of the ICC clinical team. Works with the frailty team and communicates with clinicians and administrative team members from all involved organisations within the Integrated Care Community.

2. To provide leadership and mentoring of the Frailty Coordinators working within the team.

Clinical skills

1. The Frailty Nurse(s) will be supported by the ACPs and where necessary by the GPs working in the GP practices but the Frailty Nurse(s) will also be required to work independently within their scope of professional practice.

a) Manages their clinical workload responding effectively to patient needs and ensuring patient choice and ease of access to services.

b) Functions at a high level to manage patients without input from a medical practitioner remaining within scope of a professional practice.

c) Ability to recognise the need for escalation or onward referral where appropriate.

d) Participate in Audits where required

2. Functions at a high level in relation to communication skills through nursing assessment of physical, psychological and social needs.

a) Communicates effectively, building a rapport towards a therapeutic relationship.

b) Assess, plan and implement patient centred care working where necessary with other healthcare professionals and onward referral where appropriate

3. Adapts to change within the working situation managing unscheduled/unexpected emergency situations.

a) Basic life support, severe breathing problems, serious cardiovascular problems, collapsed patients and seriously ill patients.

4. Demonstrate clinical competence in the management of patients under the care of the frailty team. To help manage and monitor patients with any chronic disease such as diabetes, asthma and chronic pulmonary disease both during periods of acute illness and regarding the ongoing management of the chronic diseases.

5. Performs safe manual handling techniques.

6. Delivers a high standard of care without discrimination, to all ages, sexes and ethnic origins of the population through continuum of life.

7. Performs investigations required to enable the team to reach a differential diagnosis using core clinical skill and competence. Initiate treatment including diagnostic investigations.

a) Recognition of abnormal vital signs observing respiratory rate and effort, heart rate rhythm and volume, temperature and blood pressure as appropriate.

b) Perform investigative procedures including, Lying and Standing Blood Pressures, Urinalysis, Blood Glucose monitoring, Venepuncture.

8. Deliver safe, effective evidence based care and advice to ease patients problems

9. Independently discharge patients from your care or refer to the appropriate health Professional or healthcare provider for further care intervention

10. Promote self-care when appropriate and encourage modification of lifestyle factors to improve patients health and wellbeing.

Organisational skills

1. Able to adapt to change in the workplace being involved in new developments in the delivery of care.

2. Ensure faulty equipment is reported and repaired and adequate for use

3. Participate and support the implementation of clinical governance, clinical audit and clinical risk assessment, setting and monitoring standards of care

a) Works within scope of professional practice and pro-actively promote the role of Frailty Nurse within the practices, and externally to key stakeholders and agencies.

2. Ensure practice and implementation of care is evidence based by adhering to local and national guidance. Promote evidence based practice using the latest research-based guidelines and the development of practice-based research

3. Maintain confidentiality of information regarding patients

4. Ensure accurate, concise documentation and consultation data in patient records in accordance with the latest NMC guidance and other pertinent standards

5. Maintains professional development in all aspects of mandatory training requirements including (but not limited to) risk awareness, health and safety, manual handling, fire safety at work, Child and Adult protection, conflict resolution and basic life support by completing on line training and also attending annual mandatory training.

6. Identify and/or participate in strategies of risk assessment to avoid or prevent clinical incidents and near misses incorporated in the clinical governance framework including:
