Job summary

The Rural West Primary Care Network (PCN) is seeking to recruit a second Health and Wellbeing Coach, to work within our PCN's multidisciplinary healthcare team and enhance our friendly, busy network of practices.

The main purpose of this role is to help make a difference to patients' lifestyles, decision making and future choices in respect of physical health and wellbeing. Working alongside our existing clinical teams, our current Health & Wellbeing Coach and Social Prescribers, this role will enhance individual patient case management, follow up and ongoing support. The Health & Wellbeing Coach will support the national 'Better Health' campaign to encourage weight loss; they will also support patients where lack of fitness/poor nutrition is contributing to or impacting on a mental or physical health condition, causing physical symptoms. In addition, the postholder will lead on the development of a range of initiatives that will benefit the health and wellbeing of our communities such as Park Runs and Park Walks.

In line with the 'WorkWell' initiative, they will support people who are disabled and those with health conditions to rejoin the workforce.

Main duties of the job

The Health & Wellbeing Coach will work closely within a multidisciplinary team with patients of low to medium complexity that will usually have one or more long-term conditions. The postholder will have a proactive approach to preventing illness in patients for whom obesity is a major risk factor for ill health in the future. They will work with a diverse range of people from different cultural and social backgrounds, so the ability to work confidently and effectively in a varied and sometimes challenging environment is essential.

Applicants will need an appropriate professional sports qualification and a valid supervision contract with a suitable professional Body. They will also have experience in health promotion and training in sports and exercise, which will include motivational coaching, behavioural change and goal setting. Also required are excellent interpersonal and communication skills and the postholder will be organised, energetic, patient and empathetic. The postholder will also preferably have experience of working in a primary care setting, ideally in health, social care or other support roles including direct contact with people, families or carers. About us

The Rural West Primary Care Network (PCN) is friendly, busy network of practices, comprising two GP Partnerships: Tadley Medical Partnership and Watership Down Health with surgeries in Kingsclere, Overton, Oakley and Tadley, Hampshire.

The post is part time (26 hours per week over 5 days) and requires travel within the Network to the surgeries of both Partnerships and communities.

The work is interesting and varied and the post holder will have the benefit of working within our supportive and friendly multi-disciplinary teams to promote excellent patient care. We are always looking to improve the quality of our services and everyone is encouraged to share great ideas. The position offers a competitive salary dependent on experience, flexibility of working and access to the NHS pension scheme.

Job responsibilities

The post holder will:

Assist both Practices within the PCN to develop a range of initiatives that will benefit the health and well-being of our patients and staff such as Park Runs and Park Walks.

Assist the clinicians within both Practices to identify patients who have been off on long-term sick or with multiple Fit Notes or disability, to work with them to help them to enter or get back into the workplace.

Identify people with long term conditions and low knowledge skills and confidence to manage their health and wellbeing.

Have responsibility for providing support to a cohort of patients who will benefit from proactive health management and care including being the single point of contact for the person or carer to simplify access and coordination of services.

Play a health coaching role; teaching and supporting patients and carers to understand and manage their own conditions and maintain an independent lifestyle through health coaching techniques to encourage patient activation.

Support the development of personalised patient care plans, liaising with the practice team, patient, carer and the complex care team as appropriate.

Proactively outreaching to patients on a regular and agreed basis.

Play an active role in regular practice MDTmeetings if required to discuss high risk and or complex patients, by gathering information and being prepared to update the team on patient progress towards goals etc as per their care plan

Map and connect community activities and resources at a locality level including supporting the development of a network of community health champions.

Support the delivery of community based public health initiatives such as physical activity, healthy eating and social connectedness.

Build and maintain strong links with the voluntary sector to network and improve partnership working.

Support delivery of systematic self-care support plans for those with COPD, diabetes, asthma and multiple long-term conditions.

Understand when it is appropriate or necessary to refer people to other health professionals/agencies.

Understand the barriers for individuals/groups in accessing support in the community and use this insight in developing community-based support, working as part of the wider social prescribing model.

Promote the service within the Primary Care Network, both for users and clinicians, building positive working relationships.

Contribute to and work with others to organise awareness raising events for services that help support people to improve their health and wellbeing.

Communicate effectively with colleagues, patients and carers so that information is shared in order to meet patients needs.

Have a key role in helping to raise the local populations awareness of the support, groups and opportunities available to assist them in achieving their health and wellbeing goals.
