Totton Primary Care Network (PCN) are looking for an inspirational, experienced clinical pharmacist who wants the opportunity to make a difference. Whilst we understand that not everyone has direct Primary Care experience, your previous pharmacy roles can be used to demonstrate that you have the ability, knowledge, drive and leadership skills necessary for this new role.
Totton PCN is in the early stages of its creation. It is formed by two forward thinking, energetic practices - Testvale Surgery and New Horizons Medical Partnership who serve a community of 37,500 patients.
Main duties of the job
The post holder will have full responsibility for the clinical pharmacist and pharmacy technicians, whilst working as part of a multi-disciplinary team in a patient facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide clinical leadership on patient centred medicines optimisation, monitoring, and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.
The post holder should be a prescriber or will be completing training to become a prescriber, they will work alongside the general practice teams.

Working with the Clinical Director, PCN Board and the Practice Clinical Leads for pharmacy, agree the strategy for Clinical Pharmacy in the PCN.
Lead a team of staff delivering a medicines management service to the practices within Totton PCN.
Deliver effective Clinical and Managerial leadership to the pharmacy team.
Provide specialist expertise in the use of medicines whilst addressing both the public health and social care needs of patients in the Totton PCN and help in tackling inequalities.
Actively support the further integration of general practice with the wider health care teams (community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
Develop relationships and work closely with other pharmacy professionals across PCN’s and the wider health and social care system.
Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, community pharmacists and anticoagulation.
Direct and manage the pharmacy team to deliver the following:
Patient facing clinical medication review, undertaking clinical medication reviews, making appropriate recommendations for the clinical team on prescribing and monitoring.
Structured medical reviews, supporting patients to take their medications, reducing waste and promote self-care.
Patient facing care home medication reviews. Work with care home staff to improve safety of medicines ordering and administration.
Management of common/minor/self-limiting ailments whilst working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GP’s or other healthcare professionals as appropriate.
Management of repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.

Essential
- Completion of an undergraduate degree in pharmacy .
- Postgraduate clinical qualification (over and above that of the 1st degree, preferably at Masters level) and/or significant relevant clinical experience to demonstrate a similar level of knowledge.
- Clinical Diploma or significant Clinical Experience.
- Membership of the Royal Pharmaceutical Society of Great Britain.
Desirable
- Hold, or working towards an independent prescribing qualification.
Knowledge and skills
Essential
- Therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities.
- Knowledge of IT systems, including ability to use word processing skills, emails and the internet to communicate and create plans and reports.
- Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions.
