Job summary

An exciting opportunity has arisen to join a Leicestershire County Primary Care Network (PCN) supporting 6 practices covering a population of approx 54,000 patients.

This role will utilise health coaching skills to support people with lower levels of patient activation to develop the knowledge, skills and confidence to manage their health and wellbeing whilst increasing their ability to access and utilise community support offers through collaborative working with the voluntary sector and Oadby & Wigston Borough Council.

Main duties of the job

This role will work collaboratively with the existing Health and Wellbeing Coach and Social Prescribing Link Workers within the PCN Health & Wellbeing Team to ensure individuals can access self-management education, peer support and social prescribing.

The post holder will undertake holistic assessments, in the GP surgery, via telephone or video call or at home if the patient is housebound, and co-design Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being through face to face and virtual contacts, including within the individuals own homes where appropriate.

JOB SUMMARY

The role is to deliver a Health and Wellbeing Coaching Service to Oadby & Wigston Primary Care Network which includes:

Wigston Central Surgery

Bushloe Surgery

Central Surgery, Oadby

South Wigston Health Centre

Rosemead Drive Surgery

Croft Medical Centre

To receive, review and act upon referrals received from GPs, primary care staff and PCN based staff. An holistic assessment will be carried out where required and patients will be coached and motivated through single or multiple sessions as appropriate.

This role will utilise health coaching skills to support people with lower levels of patient activation to develop the knowledge, skills and confidence to manage their health and wellbeing whilst increasing their ability to access and utilise community support offers through collaborative working with the voluntary sector and Oadby & Wigston Borough Council. This role will work collaboratively with the Social Prescribing Link Workers and the Care Coordinators within the PCN Health & Wellbeing Team to ensure individuals can access self-management education, peer support and social prescribing.

This role will involve travelling to meet with patients in their own home or in other agreed locations within the borough of Oadby, Wigston or South Wigston. The successful candidate will be expected to work some Saturdays and evenings to support the Enhanced Access scheme.

PRINCIPAL ACCOUNTABILITIES

1. To undertake holistic assessments, in the GP surgery, via telephone or video call or at home if the patient is housebound, and co-design Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being through face to face and virtual contacts, including within the individuals own homes where appropriate.

2. Coach and motivate individuals through single or multiple sessions to identify their needs, set goals and support them to implement their personalised health and care plan.

3. Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently and improve their health outcomes through:

Providing interventions such as self-managed education and peer support;

Supporting individuals to establish and attain goals set by the individual based on what is important to them, building on goals that are important to the individual;

Working with the PCN Health & Wellbeing Team and Local Authority partners to connect them to community-based activities which support their health and wellbeing.

4. Provide support to local community groups as part of the PCN Health & Wellbeing Team and work with other health, social care and voluntary sector providers to support the individuals health and wellbeing in a holistic manner.

5. Ensure that fellow PCN staff are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of individuals goals where an MDT is involved.

6. Raise awareness within the PCN of shared decision making and decision support tools and supporting individuals in shared decision making conversations.

7. Work with individuals with lower activation to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.

8. Explore and support access to a personal health budget, where appropriate, for their care and support.

9. Utilise existing IT and MDT channels to screen patients with an aim to identify those that would benefit from health coaching.

10. Report to the Oadby and Wigston PCN Board on a Quarterly basis.

11. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.

12. To ensure you have an understanding (appropriate to your role) of, and comply with safeguarding procedures to protect the welfare of children, vulnerable adults and other groups and individuals.

13. To comply with PCN member practices Health and Safety Policy, Data Protection Policy and to protect the health, safety and welfare of yourself and others. Whilst doing this to ensure compliance with associated legislation and regulations.

14. To work flexibly as required by the service and to take part in Oadby and Wigston PCN and relevant organisations meetings and events to promote, support and celebrate the work of the service and the agencies.

15. To be able to evidence Oadby and Wigston PCNs values at all times.

  • An appropriate health coaching or similar qualification or the ability to demonstrate an equivalent level of knowledge and understanding relating to the role.

Experience

Essential

  • A health, social care, local authority or voluntary sector background with suitable experience.
  • Demonstrate local knowledge of the services available in the area covered by the PCN.
  • Excellent holistic assessment as well as consultation skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.
  • Proven skills in collating information and data on community resources and organising these in up-to-date and accessible formats for a range of different service users from various communities.
  • Excellent record keeping skills and the proven ability to write comprehensive reports for a variety of stakeholders.
  • Good IT skills and ability to do own administration using data base, Excel and other IT packages including practice patient administration systems (EMIS Web and System1).
