Job summary

Reporting into our Nurse Leads, you will be a key part of the Clinical Team at the Croft Practice and Meadowcroft. You will be a team player, but confident working independently.

Undertake a range of nursing assessments and provide appropriate care/ treatment in conjunction with GPs according to practice policy, protocols etc.

Strive to improve standards of care and be involved in identifying practice population needs.

You must be a qualified and registered adult, child, mental health or learning disability nurse. Youll also either need to undertake further training and education or be willing to after being appointed.

Main duties of the job

You will be involved in most aspects of patient care including

  • Perform electrocardiograms (ECGs)
  • Obtaining blood samples
  • Minor and complex wound management including leg ulcers
  • Travel health advice and vaccinations
  • Initiate appropriate investigations for patients suspected to have chronic disease, i.e. diabetes, COPD, asthma, hypertension: referring to clinicians as appropriate.
  • Child immunisations and advice
  • Family planning & womens health including cervical smears
  • Mens health screening
  • Sexual health services
  • Screening and helping patients to manage long term conditions
  • Monitoring and Reporting on Complaints
  • Carry out ear examination and syringing, following appropriate training and assessment.
  • Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
  • Recognise the signs of child abuse, family violence, vulnerable adults,substance abuse, addictive behaviours, suicidal tendencies and be aware of local, referral procedure.
  • Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries
  • Ensure that current evidence based approaches to patient are standard practice

The Croft Practice cares for a mainly rural population covering an area of some 30 square miles in West Sussex, surrounding the Five Villages. As members of the Primary Health Care Team, we are committed to giving the best possible care and service to our patients. We provide medical services from our two main surgeries in Eastergate and Yapton, together with a branch surgery in Walberton.

We are rated 'Good' by CQC.

It is an exciting time at The Croft Practice as our new Medical Centre is currently being built which will enable us to continue to provide excellent services to our community.
