Job summary
Rectory Meadow Surgery is looking to expand its clinical team and is looking for an experienced Nurse Practitioner or Advanced Nurse Practitioner . We are open to full or part time working for the right candidate.
The aim is to provide high quality care to patients of Rectory Meadow Surgery with the focus on providing additional nursing clinics i.e. chronic disease, or minor illness and potentially assisting in home visiting for vulnerable patients with acute or long term condition health needs.
The aim of the post is to utilise specialist clinical skills and knowledge to:
Consult and effectively manage patients in the clinic setting and in their own homes with the support of a GP
Provide evidenced-based clinical treatments for patients with chronic or long term conditions or acute presentiations
Desirable Skills (Or motivated to working towards)
Long term conditions - respiratory / spirometry
Diabetes management
Minor ailments / illnesses
Experience consulting patients - both in the clinic setting and at home
Independent Prescribing Qualification (or motivated to work towards)
This post will add an additional tier to the practice clinical structure, the post holder(s) will be expected to conduct themselves in a manner appropriate to the role and as a senior representative of Rectory Meadow Surgery.
The post will be subject to a 3 month probationary period.
The post will be supported with professional development and mentoring where required.
Main duties of the job
Key tasks & responsibilities
Depending on key skills and areas of expertise:
Assist with chronic disease management of patients including routine reviews and exacerbations management
Assist with clinical triage of patients both online or in clinic, potentially conduct own minor illness/ injuries
Ensure good use of resources, including time, prescribing and other consumables
To monitor home visit requests and make initial assessments on the most appropriate person to visit, allocating and facilitating this process
Use available clinical systems, both static (on-site) and mobile to enter information into patients notes
Refer to GPs for prescribing needs and advice when required, utilising PGD/PSD approval where necessary
Maintain close working relationships with PCN team especially pharmacists, health and wellbeing coaches and social prescriber
Proactively identify patients who may benefit from case management or additional input
Participate in advanced care planning where appropriate, liaising with the patients usual GP
Carry out appropriate onsite consultations when time allows i.e. supporting and contribute to current nursing team dependent on skills and experience
Play an active role in personal and team development across the multidisciplinary team.

We are a large GP Practice situated in Old Amersham providing General Medical Services to over 10,600 patients. We have 4 GP Partners, 3 Salaried GPs and as a training practice sometimes up to 3 GP Trainees. The GPs are supported by a strong nursing team with a wealth of general nursing and chronic disease management experience.
