About the Role

St Helens South Primary Care Network (PCN) is seeking an experienced and highly motivated Band 6 (band 5 considered) Mental Health Nurse to join our multidisciplinary primary care team.

Main duties of the job

This is an exciting opportunity to work at the forefront of integrated primary care, delivering high-quality mental health assessment, intervention and care coordination for adults with a range of mental health needs. You will work autonomously within your scope of practice while contributing to service development.

Job responsibilities

Key Responsibilities

Clinical Care

  • Provide comprehensive mental health assessments, formulation and care planning for patients presenting in primary care
  • Deliver evidence-based interventions for common mental health conditions (e.g. anxiety, depression, PTSD, severe mental illness)
  • Undertake risk assessments, crisis management and safeguarding interventions
  • Manage a defined caseload and work autonomously with supervision as required
  • Liaise with GPs, multidisciplinary teams, and secondary mental health services to ensure coordinated care
  • Support long-term condition management for patients with serious mental illness

Leadership and Service Development

  • Provide clinical supervision, mentorship and support to junior mental health staff and trainees
  • Contribute to service development, quality improvement initiatives and PCN mental health pathways
  • Participate in audits, policy development and clinical governance activities
  • Act as a mental health resource and advisor within the PCN

Education and Health Promotion

  • Deliver psychoeducation to patients, carers and families
  • Support mental health promotion, prevention and early intervention initiatives across the PCN population

  • Essential Criteria
  • Registered Mental Health Nurse (RMN) with current NMC registration
  • Significant post-registration experience in mental health (community, primary care or secondary care)
  • Experience of autonomous clinical practice and caseload management
  • Strong assessment, risk management and safeguarding skills
  • Excellent communication, interpersonal and organisational skills

Desirable

  • Desirable Criteria
  • Experience working in a Primary Care Network or GP practice setting
  • Postgraduate qualification or training in mental health, CBT-informed interventions or psychological therapies
  • Experience in leadership, supervision or mentorship
  • Non-medical prescribing qualification or working towards
