Job summary

We are looking for enthusiastic, flexible and self-motivated Physician Associates to join a newly formed Frailty team in St Helens South Primary Care Network. You will work in a variety of settings including Community GP practices and Domiciliary settings.

A Special interest in Frailty would be a strong advantage.

We currently have a small team of Physician associates and hoping to expand our workforce.

Please note this role is not based at Holly Bank Surgery. Please do not contact Holly Bank Surgery in regards to this advert. The role is employed by St Helens Rota and Managed by St Helens South PCN.

Main duties of the job

Daily activities will consist of:

  • Liasing with Care Homes within the Network and providing a link with GP and Care Home
  • Participating in Weekly MDT's
  • Making MDT Notes on Emis
  • Deciding which Patient to discuss in advance
  • Keeping up to date with Bloods/results
  • Examining Patients as required

Job responsibilities

Under supervision of the GP in the practice, the post holder will provide care for the presenting patient from focused history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the service. The post holder will demonstrate critical thinking in the decision-making process. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. The post holder will provide a holistic and clinical service, with support and supervision of GPs, implementing agreed management plans and following approved protocols as appropriate.

  • Degree at 2:1 level or above
  • Postgraduate Diploma/Master in Physician Associate from an accredited UK University

Desirable

  • Member of Physician Associate Managed Voluntary Register
  • Evidence of Continuing Professional Development activities
  • Basic Life Support

Experience

Essential

  • Advanced clinical practice skills
  • Clinical examination skills
  • Management of patients with common acute medical conditions
  • Management of patients with long-term conditions
  • Knowledge of NHS
  • Understanding of the current issues and challenges facing primary care

Desirable

  • Management of patients with complex needs
  • Experience of preventative medicine and chronic disease management
  • Experience of working to achieve standards within the Quality and Outcome Framework (QOF)
