Job summary
We are seeking 2 Care Co-ordinators to join a newly formed Frailty Team within St Helens South Primary Care Network. The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). Leadership will be provided by a Newly Appointed ANP. Please note this role will be employed by St Helens Rota and Managed by St Helens South PCN. Main duties of the job
The Role of the Care Co-ordinator may be perfect for you, if you are organised, good at building relationships, caring and compassionate. You will be responsible for facilitating case management services for patients. We are looking for candidates who possess: NVQ Level 2 in Health Care or Equivalent Experience Provide Clinical Intervention for Patients as directed by registered Nurses. Knowledge of Safeguarding Vulnerable Adults Procedure and Competent in use of Clinical Skills. Knowledge of Health Checks and ability to perform Clinical Observations.
