Job summary
Under supervision of the GPs in the practice, the post holder will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the service. The post holder will demonstrate critical thinking in the decision-making process. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. The post holder will provide a holistic and clinical service, with support and supervision of GPs, implementing agreed management plans and following approved protocols as appropriate and will be supervised by a member of the clinical team. Will run a duty clinic along side the Duty doctors. Will help manage clinics and aid with supporting all duty staff.
Main duties of the job
- Act upon alerts & reminders during a routine consultation.
- Diagnose & formulate a management plan, with reference to a health professional.
- Run minor illness clinics in the A.M which will include face to face consultations & telephone consultations
- Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment & referral
- Adhere to the practice prescribing formulary, local & national guidelines.
- Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team.
- Improve patient & carer understanding of confidence in & compliance with their medication.
- Order bloods using ICE.
- Use of templates throughout the system where required.
- Promote safe use of medication, reporting of medicines-related incidents & pro-actively preventing safeguarding incidents.
- Observe the principles of clinical governance when developing policies, protocols, guidelines, SOPs & procedures.
- Support practice prescribing budgets by on-going monitoring, audit & analysis of drug use & support practices in reviewing, developing and enhancing cost-effective prescribing. This includes staying aware of latest QIPP & other cost targets
- Record clear & contemporaneous IT based consultation notes to agreed standards;

About us
Located in purpose built building in the heart of Leighton Buzzard town.
Established Clinical team comprising of two GP Partners, one Lead Nurse, five Salaried GPs, two Practice Nurses, one Nurse Associate, two Health Care Assistants. This will be in addition to one Senior Practice Pharmacist, and one advanced nurse practitioner. We are supported by a diverse PCN team.
List size of 12,000 patients.
Clinical system SystmOne.
We use Ardens template toolkit which provides structure and standardisation for clinical consultations.
Remote consultations offered where appropriate using eConsult and ACCURX.
