Duties and Responsibilities

In accordance with the practice timetable, as agreed, the post-holder will make himself

available to undertake clinical sessions.

You will be required to:

• Act upon alerts (e.g. QOF) and reminders during a routine consultation.
• Diagnose and formulate a management plan, with reference to a health professional where appropriate.
• Follow up the care of patients as appropriate.
• Run minor illness clinics in the A.M which will include face to face consultations and telephone consultations/advice/triage.
• Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate.
• Provide appropriate preventative health care and advice.
• Provide medical support as required to patients referred by nursing staff.
• Adhere to the practice prescribing formulary, local and national guidelines.
• Under supervision of the GPs in the practice, make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
• Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team.
• Improve patient and carer understanding of confidence in and compliance with their medication.
• Identify blood tests, recalls and reviews using established protocols and forward these to the appropriate clinician.
• Order bloods using ICE.
• Use of templates throughout the system where required.
• Check own pathology results where applicable.
• Support and respond to medicines-related queries from medical and nursing colleagues and from patients and relatives via face to face, via phone or via email. Queries can include issues with interactions or ADRs, availability of stock, swallowing difficulties, cost-effectiveness, urgent requests or extra supplies.
• Promote safe use of medication, reporting of medicines-related incidents and pro-actively preventing safeguarding incidents.
• Observe the principles of clinical governance when developing policies, protocols, guidelines, SOPs and procedures.
• Support practice prescribing budgets by on-going monitoring, audit and analysis of drug use and support practices in reviewing, developing and enhancing cost-effective prescribing. This includes staying aware of latest QIPP and other cost targets.

Contributes and participates in audits, evaluation and clinical standard setting within the Practice Salisbury House Surgery.

Accurate and timely summarising of patient records and read-coding patient data

Complete all required paperwork for legal and administrative purposes in accordance with relevant standards

Ensure that all practice policies are fully implemented

Work in accordance with all governance and internal systems relating to (but not limited to) the management of clinical data and systems

• Record clear and contemporaneous IT based consultation notes to agreed standards;
• Deal with Practice notes, review and file patient lab. results and discuss with GP where necessary;
• Collect data for audit purposes.
• PM Home Visits (Maximum of 2 Patients) when required.
• PM clinics for Long term conditions.

Other responsibilities with the Practice:

• Supporting the practice staff and responding to requests for advice and assistance from the practice reception, secretarial and nursing staff.
• Attend prescribing, palliative care, clinical and practice meetings.
• You will be expected to take part in clinical meetings on a weekly basis
• Provide additional enhanced services to the practices that meet the needs of the practices patient population and maximise the income streams to the practice
• Undertake additional training where necessary to provide these enhanced services
• Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety, annual QOF Assessment of the Practice.
• Meet timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding (e.g. Quarterly Enhance Services returns, annual QOF audit etc.)

General Duties

Administration and professional responsibilities

• Participate in the administrative and professional responsibilities of the practice team
• Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
• Take a share of test results and hospital referral letters
• A share of QOF administrative related reviews
• Participate in patient care planning
• Reviewing a share unplanned hospital admissions and A+E attendances
• Ensure the clinical computer system is kept up to date with accurate details recorded and amended
• Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
• Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
• Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
• Attend and participate in practice meetings as required
• Home visits

The post holder may be required to fulfil other duties, as agreed with the practice manager to

meet the needs of the organisation. This may involve travel to other sites within the

organisation

Training and personal development

• Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.
• Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate
• Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met.
• If it is necessary to expand the role to include additional responsibilities, full training will be given.
• Develop and maintain a Personal Learning Plan

Liaison

• As well as with the partners and nursing team there is a need to work closely with reception, office and other staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team.
• There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care

Meetings

• It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.

Person Specification

Qualifications

Essential

• Pharmacy degree
• Registered with the GPhC
• Independent prescribing qualification
• Evidence of commitment to continuing professional
• development

Desirable

• Previous experience in a GP practice and/or hospital environment.
