Job summary
The Health & Wellbeing Coach will empower people particularly those living with long term conditions to take control of their health. Working under the Additional Roles Reimbursement Scheme, you will provide one to one and group support that blends personalised goal setting, behaviour change coaching. In doing so, you will champion preventative healthcare and help reduce health inequalities across our community.
Main duties of the job
Personalised Health Coaching deliver tailored one to one and group sessions, using motivational interviewing to set and achieve patient centred goals.
Team Based Integrated Care collaborate with GPs, nurses, social prescribers and community partners to ensure coordinated, holistic support, referring and signposting as appropriate.
Data Driven Record Keeping maintain accurate patient records on SystmOne, monitor progress, and provide outcome reports to evidence impact and inform quality improvement.
Quality, Governance & Professional Growth uphold safeguarding, information governance and infection control standards while engaging in ongoing supervision and CPD to keep practice evidence based.

- Level2 or above qualification in health coaching, public health, fitness or a related discipline.
Desirable
- Recognised qualification in physical activity or exercise instruction (e.g. REPsLevel3 or equivalent).
Other Requirements

- Proficient in motivational interviewing and behaviour change models.
- Excellent communication and interpersonal skills, able to build rapport with diverse populations.
- Personal attributes: empathetic, non judgemental, organised, self motivated and a collaborative team player.
- Enhanced DBS clearance plus full UK driving licence and access to a vehicle (for community based travel).
