Job summary

We are a small friendly surgery in Croxley Green and part of the larger Grand Union Primary care network(GUPCN). If you join us you will benefit from working with a close nit supportive team and strong collaboration with GUPCN. As a practice we strive to deliver services to improve health and wellbeing of individual and communities.

We are looking for a forward thinking practice nurse with experience in long term health conditions, who has a passion for helping patients and great sense of humour (a good baker helps!). You need to be resilient, confident, well organised and able to multi-task. NMC registration a must.

Main duties of the job

Core responsibilities will be childhood and adult immunisations, wound care, contraception, cervical screening, spirometry and long term condition management such as Diabetes, COPD and Asthma.

You would also be able to offer general treatment room services such as BP, ECG, venepuncture and health promotion.

The role offers you the opportunity to grow and develop, you would be responsible for room stocking, ordering and mentoring our HCA.

Baldwins Lane Surgery is a small surgery in Croxley Green. Our clinical team consists of 2GP partners, 1 salaried GP, Clinical Pharmacist, HCA/phlebotomist, Practice nurse who are supported by a friendly team of reception and admin staff.

We work hard however staff wellbeing is paramount with the partners who engender a great team spirit amongst the staff and emphasise a good work life balance is maintained in the team.

Job description

Job responsibilities

The following are the core responsibilities secondary requirements are detailed in job description.

a. To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager

b. To produce the IPC annual statement in conjunction with the practice manager

c. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

d. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

e. Develop, implement and embed health promotion and well-being programmes

f. Implement and evaluate individual treatment plans for chronic disease patients

g. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health

h. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

i. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF

j. Provide wound care (ulcer/Doppler etc.) to patients

k. Provide travel medicine services

l. Request pathology services as necessary

m. Process pathology results as required

n. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

o. Maintain accurate clinical records in conjunction with extant legislation

p. Ensure read codes are used effectively

q. Maintain chronic disease registers

r. To review patient medications to enhance compliance

s. Develop, implement and embed well woman clinics

t. Develop, implement and embed well man clinics

u. Chaperone patients where necessary

y. Recognise, assess and refer patients presenting with mental health needs

z. Implement vaccination programmes for adults and children

aa. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

bb. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

cc. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

dd. Support the clinical team with all safeguarding matters, in accordance with local and national policies

a. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

b. To ensure SNOMED codes are used effectively by all members of the nursing team.

c. To assist in performing routine tasks related to patient care as asked by the GPs and Practice Manager such tasks may include but not limited to

- Wound care

- Removal of sutures

- ECGs

- Smear tests

- Urinalysis

- Peak Flow readings

- Venepuncture

- Blood pressure readings fitting of 24-hour bp machine

- Routine immunisations including childhood immunisations.

- Travel vaccinations

- Maintaining GP and nurses room, stocking and rotating items as required.

- Ensure GP doctors bags are regularly checked and stocked.

- Undertake housekeeping duties including general tidiness and cleanliness of nurses treatement room and doctors consulting rooms

- Ordering of vaccinations to maintain stock levels

- Participation in administrative systems in the practice.

d. Required to undertake home visits if the needs arrive.

e. Participate in the delivery of COVID-19 vaccine whether on or off site.

f. Any other delegated duties appropriate to the post at this site or any other designated site within our Primary Care Network (PCN)
