Job summary

We are looking for someone to enhance our existing clinical team with expertise in clinical medicines management, face to face and remote structured medication reviews, long term condition management, management of medicines on transfer of care and systems for safer prescribing.

This is an exciting opportunity for the right candidate to work as part of the team and manage quality improvement and clinical audits, as well as some aspects of the Quality and Outcomes framework and local prescribing schemes.

There is also the opportunity to be involved in research.

Main duties of the job

Carry out patient-facing Long Term Condition clinics and medication reviews.

Management of common/minor/self-limiting ailments.

Complete all medication switches and reconciliations with discharge letters.

Fulfill contractual requirements of PCN DES and local prescribing schemes.

Support team members and patients with queries about medicines, signposting to appropriate health professionals.

Work with GP's on repeat prescribing scheme / implement national and local guidance.

Work with GP's on service development.

Ensure medicines safety and quality improvement.

About us

We provide primary care services to approximately 6800 patients from our premises in Padgate, Warrington and our branch surgery, a short distance away in Woolston Neighbourhood Hub. We are a supportive practice with a full complement of Clinical staff which includes three GP Partners, three Salaried GP's, two practice Nurses and a GP Assistant/ Health Care Assistant, as well as an established patient advisor and administrative staff. We also a number of ARRS roles in post to support our team. We are a dynamic, progressive, forward-thinking and very friendly team, as well as being a training and research practice. We strive to provide excellent clinical care within a mutually supportive environment for professional development.

Patient-Facing Long Term ConditionClinics/Reviews

  • See (where appropriate) patients with single ormultiple medical problems where medicineoptimisation is required (e.g. COPD, asthma).Review the on-going need for each medicine, areview of monitoring needs and an opportunity tosupport patients with their medicines takingensuring they get the best use of their medicines(i.e. medicines optimisation). Make appropriaterecommendations formedicine improvement.
  • Undertake clinical medication reviews with patientsand produce recommendations on prescribing andmonitoring.

Management of Common/Minor/Self-limiting Ailments

  • Managing caseload of patients withcommon/minor/self-limiting ailments while workingwithin a scope of practice and limits of competence. Signposting to community pharmacy and referringto GP's or other healthcare professionals whereappropriate.

Medicines Support

  • Provide patient facing clinics and telephone support for those with questions, queries and concerns about their medicines in the practice.

Medicines Information to Practice Staffand Patients

  • Answers relevant medicine-related enquiries fromGP's, other practice staff, other healthcare teams(e.g. community pharmacy) and patients withqueries about medicines.Suggesting and recommending solutions.Providing follow up for patients to monitor the effectof any changes.

Unplanned Hospital Admissions

  • Review the use of medicines most commonlyassociated with unplanned hospital admissions andreadmissions through audit and individual patientreviews.Put in place changes to reduce the prescribing ofthese medicines to highrisk patient groups.

Management of Medicines at Dischargefrom Hospital

  • Reconcile medicines following discharge fromhospitals, intermediate care and into care homes,including identifying and rectifying unexplainedchanges and working with patients and communitypharmacists to ensure patients receive themedicines they need post discharge.
  • Set up and manage systems to ensure continuity ofmedicines supply to highrisk groups of patients.

Signposting

  • Ensure that patients are referred to the appropriatehealthcare professional for the appropriate level ofcare within an appropriate period of time e.g.pathology results, common/minor ailments, acuteconditions, long term condition reviews etc.

Repeat Prescribing

  • Produce and implement a practice repeatprescribing policy.
  • Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeatprescriptions and reviewing medicines reachingreview dates and flagging up those needing areview.
  • Ensure patients have appropriate monitoring testsin place when required.

Medicines Quality Improvement

  • Undertake clinical audits of prescribing in areasdirected by the GP's, feedback the results andimplement changes in conjunction with the practiceteam.

Medicines Safety

  • Implement changes to medicines that result fromMHRA alerts, product withdrawal and other localand national guidance.

Implementation of Local and National Guidelines and Formulary Recommendations

  • Monitor practice prescribing against local standards and make recommendations toGP's for medicines that should be prescribed byhospital doctors (red drugs) or subject to sharedcare (amber drugs).
  • Assist practices in seeing and maintaining apractice formulary that is hosted on the practicescomputer system.
  • Auditing practices compliance against NICEtechnology assessment guidance.

Service Development

  • Contribute pharmaceutical advice for thedevelopment and implementation of new servicesthat have medicinal components (e.g. advice ontreatment pathways and patient informationleaflets).

  • Mandatory registration with General Pharmaceutical Council
  • Masters degree in pharmacy (MPharm) (or equivalent)

Desirable

  • Working towards/intent of gaining independent prescribing qualification.

Experience
