Tower Hamlets Public Health
Team is launching a new programme that aims to reduce health inequalities and
prevent long term conditions across the borough through a community-centred
approach. This Public Health team & NHS North East London funded programme,
will be delivered by primary care teams in partnership with Public Health, Voluntary
and Community Sector Organisations and Local Residents.
We
are looking for a motivated individual for this role in the South East of the
Borough. If you have experience of working in local communities and
are interested in co-developing community-centred approaches to the prevention
of long-term conditions, then we
are keen to hear from you!
The
role will involve working with community groups, residents, GPs and a range of
stakeholders to improve specified health and wellbeing outcomes for our local communities.
We feel building strong,
trusting partnerships with residents, agencies and services will empower
communities to identify and develop community capacity and resilience to
address health inequalities.
Candidates
should demonstrate:
A track record in delivering
community development with in challenged and diverse communities through either
the NHS, local government or voluntary/community sectors.
Knowledge of and commitment
to the principles of asset-based community development
Understanding and promotion
of the values of coproduction and inclusion.
Main duties of the job
Engage with residents and stakeholders in the
locality.
Organise Events to facilitate community engagement.

With the local team Develop a Participatory
Budgeting Programme
Monitor and evaluate the programme as you go along.
About us
Over the last 12 years both Health Networks
(P&L and HIP) have worked together
to provide innovative community facing health services to the diverse
population of the southeast of Tower Hamlets.
The area has a high number of patients with
long term conditions, and some of the highest concentration of Diabetes and
COPD across East London. The Locality like others has pockets of high
deprivation though is unique as it is juxtaposed against the wealth and
opulence of Canary Wharf - bringing a stark contrast.
The two Networks have a successful track record of
working together.
We ran a Health Trainers and a Communities Driving Change
programme CDC
V4 - YouTube across the Locality over the last
10 years.
Key to our success has been the partnership with local

At the start of COVID-19 lockdown the Networks came
together with local community groups to create the SE Health Link Team to
support our most at risk patients, and provided frontline services such as food
and prescription deliveries.
In 2021 both Networks formed an alliance to provide COVID-19
Vaccine boosters to our community from two hubs at Barkantine and at Newby
Place. We have since then delivered over
20000 vaccinations to our local residents.
We currently run Extended GP services on weekday evenings
and weekends in three venues across the Locality.

Post Title: Locality Community Health Facilitator
Reporting to: Locality Wellbeing Manager
Accountable to: SE Locality Health & Wellbeing Committee
Overall purpose of the post
- Develop community-centred approaches to prevention of long-term conditions by working with community groups, residents and a range of stakeholders to shape preventive services and activities, increase participation in preventive activities and improve specified health and wellbeing outcomes; especially communities that are not currently engaging in/accessing preventive activities.
- Build strong, trusting partnerships with communities, agencies and services and empower communities to identify and develop community capacity and resilience to address health inequalities.
Specifically the post will:
Work with a range of stakeholders to implement asset-based community and stakeholder engagement and development for long term condition prevention (using CSEAD and Well Communities methodology)
Support the development and implementation of local planning based on the above
Support community-led participatory budgeting and action to address identified barriers and facilitators in line with local plans
Collect data as required for the monitoring and evaluation of the programme
Management Structure
The Locality Community Facilitator will be responsible to the Network Manager (or equivalent) and accountable to the Locality Health and Wellbeing Committees.
Key responsibilities of this Post
1. Engage with residents and stakeholders in the locality
- Work in partnership with local organisations, statutory services, voluntary agencies and residents, using a high level of relationship management to create strong and enduring partnerships around long term condition prevention and tackling health inequalities
b. Work with local communities to shape preventive services and activities, increase participation in preventive activities and strengthen their capacity to include and support people who are vulnerable and socially excluded/marginalised
- Maintain the strong and productive relationships and partnerships with and between residents and local services
- Manage and support the development of local resident health champions /volunteers, including reward and recognition provision
- Promote equality and value the diversity of individuals
2. Implement an asset-based community development approach in the locality
a. Support the local delivery team to use the Community and Stakeholder Engagement, Assessment and co-Design (CSEAD) methodology to engage priority communities, map assets and identify priority areas for action
b. Participate in briefings, training and action learning about the CSEAD methodology
c. Support local volunteer health champions to strengthen project outreach in their communities
d. Support the coproduction of local action plans for each area; including shaping and further developing preventive services and activities
e. Plan, attend and co-ordinate programme meetings, workshops and events
f. Support the facilitation of workshops and events
3. Action planning and participatory budgeting
a. Support the implementation of initiatives against co-produced action plans using agreed methodologies
- Administer, manage and monitor participatory budgets.
c. Collect specified data/information and review progress quarterly and refresh the action plan annually.
4. Communication and community insight
- Develop culturally appropriate, accessible and socio-economically appropriate communication materials as required
- Develop and maintain a clear understanding of local community strengths, resources, connections, gaps and opportunities to develop creative solutions
- Gather information, insight and intelligence about health inequalities and long-term conditions in local communities.
- Provide regular updates to the locality team and Portfolio programme officer/public health project manager.
5. Team working at locality/PCN/ borough level
- Co-ordinate and support the local delivery team to implement the programme focusing on specified target groups.
b. Liaise with local community connectors, social prescribers and other similar non-clinical roles.
- Work with the Operational Group to ensure a common approach across the borough/programme and share learning
- Work closely with public health programme managers and other local team members
- Co-ordinate and minute local meetings as required.
6. Monitoring and evaluation
a. Ensure good quality data/information is collected at community level and proper monitoring records are maintained for all eligible persons in the local area through use of an endorsed data system, providing information and data for reporting purposes
b. Respond to requests for information.
c. Assist in the collation of statistical information in order to provide evidence that the community project plan is meeting identified outcomes
d. Monitor local programme progress against agreed outcomes and undertake quarterly reporting
e. Work closely with programme evaluators and respond promptly to requests from evaluators, as required.
7. Manage and organise own time and activities
a. Keep adequate records of the work undertaken as agreed with line manager
b. Alert line manager to any issues in work (including concerns about individuals or work in the community)
c. Take an active part in development of own knowledge and skills in community-based development
d. Seek advice and support as and when appropriate
e. Attend all relevant and required training and comply with Network arrangements for continuing professional development
f. Work flexibly as required including out of core hours, agreeing time off in lieu (TOIL) in advance with the line manager
