Job summary
The Poplar and Limehouse Health and Wellbeing Network is a
partnership of 4 local general practices, community care teams, and local third-sector
providers set up in Tower Hamlets over 10 years ago. It became a Primary Care
Network in 2019 and it continues to strive to deliver the best outcomes for its
patients through a joined-up approach, quality access and equality of GP
services. The successful candidate will be based in one of the practices and join
a growing team of health professionals dedicated to provide integrated patient-centred
care.
You will show courageous and critical thinking and are a caring, compassionate and committed experienced nurse. You will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills for patients within the practice.You will work collaboratively with the team to meet patients' needs and provide leadership as required.

Main duties of the job
Assess,
diagnose, plan, implement and evaluate treatment/interventions and care for
patients (pts) presentingwith an
undifferentiated diagnosis and pts with complex needs
Clinically
examine and assess pts' needs from a physiological and psychological
perspective, planclinical
care
Proactively
identify, diagnose and manage treatment plans for pts at risk of developing a
long-termcondition
Diagnose and
manage acute and chronic conditions, integrating both drug- and non-drug-basedtreatment
methods into a plan
Prescribe
and review medication for effectiveness, appropriate to pt needs and in
accordance with evidence-based practice and national and practice protocols,
and within scope of practice
Provide
information and advice on prescribed or over-the-counter medication on
medication regimens, side-effects and interactions
Work with pts to support compliance with and adherence to prescribed treatments
Support
patients to adopt strategies that promote healthy lifestyles, and apply
principles of self-care
Support and
manage health needs of women presenting for family planning, cervical cytology
or sexual health consultation
Assess and
refer pts presenting with mental health needs according to the NSF for Mental
Health
Implement
and participate in vaccination and immunisation programmes for both adults and
children
Meet the needs of patients presenting for opportunistic wound care

Whilst based in one of our practices, you will be joining the PCN's multi-disciplinary team of
17, and a team of over 150 staff across
the four practices.

and provide seamless integrated care to individuals wherever their first point
of contact in the network.

The Role
You will
