Job responsibilities

1.To assist with the assessment, planning and implementation of agreed care in relation to the management of long-term conditions, including annual and follow up health reviews specifically for this post, the following (some of and not necessarily all of -

a.Diabetes

b. Asthma

c. COPD

d. Coronary heart disease

e. Heart Failure

f. Hypertension Management

g. Rheumatoid arthritis monitoring

h. Family planning

i. QOF areas learning disability checks and mental health reviews.

(Additional training will be arranged if required in these areas.)

2. To advise patients on general health care with referral to GP as necessary.

3. Perform a holistic assessment of patients attending for a range of Practice nursing duties, including (but not exclusive)

a. Cervical screening

b. Wound care / ulcer care/Doppler including compressions bandaging

c. Removal of Sutures

d. Well man/woman checks

e. Travel advice and immunisation

f. Warfarin monitoring

g. Contraception and womens health advice, including HRT.

h. Venepuncture

i. Routine immunisations/Childhood immunisations

j. Chaperoning and assisting patients where appropriate who are being examined by another clinician

k. Assisting with minor surgery and coil fittings, as requested

l. Requesting pathology tests, for example urine culture, swabs

m. To ensure the responsible use of all emergency medical equipment.

n. To maintain a high standard of cleanliness in the clinic rooms and adhere to infection control measures at all times.

o. Involvement (design and redesign) of EMIS template writing for areas relevant to the role.

p. Full engagement in Quality and Outcomes Framework (QOF) and any other centrally set measuring tools, and participation on a daily basis of this work within consultations.

q. Cleaning and stocking of nursing rooms, including checking of stock dates, and keeping areas tidy.

r. Maintaining and cleaning equipment used by the nurses and GPs as requested.

s. Participation in administrative systems in the Practice

t. Monitoring and ordering stock if required for items such as vaccinations and dressings

Essential

• Excelling communication skills (written and oral)
• Time management and the ability to work to clinic times/other deadlines
• Ability to deal with crises, interruptions and assess the urgency of some conditions
• Ability to pay attention to detail
• Good organisational skills
• Excellent interpersonal skills

Desirable

• Good treatment room skills - see attached spec.

Qualifications

Essential

• Good standard of education
• eg GCSEs A-C, A Levels A-CRegistered Nurse with current NMC registration
• Can provide evidence of continuous professional development (CPD)

Desirable

• Educated to degree level or equivalent
• Certificate/diploma in a chronic disease area or currently undertaking one
• Family Planning Certificate
• CPR/anaphylaxis training undertaken in the last 12 months

Experience

Essential

• Experience of working as part of a multidisciplinary team

Desirable

• Practical experience of working with computers
• Experience of working as a practice nurse within General Practice
