Job summary

An exciting opportunity has arisen for a Practice Nurse to join our team at Cantilupe Surgery in Hereford City from September 2024. This will be a part time role 2 to 3 days per week, based at both Cantilupe and Hampton Dene surgeries.

Your role with be to provide good treatment room services to our patients, and have a good all round knowledge of chronic disease clinic work.

Main duties of the job

See attached job description - Work will include the usual GP surgery treatment room work, to include, immunizations (adult and child) , cervical cytology, ECGs, travel screening and imms, contraception advice, INRs, and dressings including complex leg dressings.

You will be expected to have evidence of training to provide care for patients with all long term conditions. A special interest and/ or qualification in diabetes and/or heart disease would be desirable but not essential and training would be offered for the right candidate.

About us

Cantilupe Surgery is a large city centre training practice with 12630 patient, based over 2 sites within the city. Parking is available at both sites for staff.

We achieved a 'Good' CQC rating (2024) and were awarded 'General Practice of the year' at the 2023 Herefordshire Health and Social Care awards, voted for by our patients.

We have a large GP (6) and on the day ANP/Paramedic team supported by Practice nurses and HCAs as well as a pharmacy team and other attached PCN staff including, wellbeing, safeguarding and care home support workers.

You will be very well supported by our lead Nurse, who will ensure you are fully compliant with all aspects of work expected.

We have an active PCN with good peer support from both within the team and via the network.

Job responsibilities

1.To assist with the assessment, planning and implementation of agreed care in relation to the management of long-term conditions, including annual and follow up health reviews specifically for this post, the following (some of and not necessarily all of -

a.Diabetes

b. Asthma

c. COPD

d. Coronary heart disease

e. Heart Failure

f. Hypertension Management

g. Rheumatoid arthritis monitoring

h. Family planning

i. QOF areas learning disability checks and mental health reviews.

(Additional training will be arranged if required in these areas.)

2. To advise patients on general health care with referral to GP as necessary.

3. Perform a holistic assessment of patients attending for a range of Practice nursing duties, including (but not exclusive)

a. Cervical screening

b. Wound care / ulcer care/Doppler including compressions bandaging

c. Removal of Sutures

d. Well man/woman checks

e. Travel advice and immunisation

f. Warfarin monitoring

g. Contraception and womens health advice, including HRT.

h. Venepuncture

i. Routine immunisations/Childhood immunisations

j. Chaperoning and assisting patients where appropriate who are being examined by another clinician

k. Assisting with minor surgery and coil fittings, as requested

l. Requesting pathology tests, for example urine culture, swabs

m. To ensure the responsible use of all emergency medical equipment.

n. To maintain a high standard of cleanliness in the clinic rooms and adhere to infection control measures at all times.

o. Involvement (design and redesign) of EMIS template writing for areas relevant to the role.

p. Full engagement in Quality and Outcomes Framework (QOF) and any other centrally set measuring tools, and participation on a daily basis of this work within consultations.

q. Cleaning and stocking of nursing rooms, including checking of stock dates, and keeping areas tidy.

r. Maintaining and cleaning equipment used by the nurses and GPs as requested.
