Main duties of the job
Your Role: As a primary care ACP, youll play a key role in managing minor illnesses, common GP surgery presentations, and long-term conditions. Youll work both independently and collaboratively, ensuring our patients receive the highest standard of care.
Key Responsibilities:
- Assess, diagnose, and develop effective clinical management plans.
- Provide face-to-face or telephone consultations for undifferentiated patient presentations.
- Manage acute and chronic conditions with confidence and compassion.
- Offer patient education and support, promoting self-management and healthy choices.
- Contribute to the delivery of practice policies and procedures.
