Main duties of the job

Your Role: As a primary care ACP, youll play a key role in managing minor illnesses, common GP surgery presentations, and long-term conditions. Youll work both independently and collaboratively, ensuring our patients receive the highest standard of care.

Key Responsibilities:

  • Assess, diagnose, and develop effective clinical management plans.
  • Provide face-to-face or telephone consultations for undifferentiated patient presentations.
  • Manage acute and chronic conditions with confidence and compassion.
  • Offer patient education and support, promoting self-management and healthy choices.
  • Contribute to the delivery of practice policies and procedures.
