Job summary
Chatfield Health Care is seeking a motivated and compassionate Practice Nurse to join our friendly and supportive primary care team. This is a part-time role working Tuesdays, Thursdays, and Fridays, providing high-quality nursing care to our patient population.
You will play a key role in delivering evidence-based care, supporting long-term condition management, health promotion, and preventative services within a busy GP practice.
Main duties of the job

Job Purpose
The Practice Nurse will work as part of the primary care team at Chatfield Health Care, providing high-quality, evidence-based nursing care to the practice population. The post holder will deliver a range of clinical services, including chronic disease management, health promotion, screening, and treatment, working within their professional scope of practice.
The role supports the practice in delivering safe, effective, and patient-centred care, contributing to improved health outcomes and service delivery.
Key ResponsibilitiesClinical Practice

Deliver high-quality nursing care to patients within agreed protocols and scope of practice

Undertake chronic disease management, including asthma, COPD, diabetes, and hypertension

Carry out cervical cytology (smear tests) in line with national guidelines

Administer immunisations and vaccinations in accordance with national and local programmes

Perform wound care, dressings, and management of minor injuries where appropriate

Undertake NHS Health Checks and other screening programmes

Carry out phlebotomy, blood pressure monitoring, and ECGs
-
Provide contraception advice and support (where appropriately trained)

Promote health and wellbeing through patient education and lifestyle advice
