Job summary
Pelham Medical Group is looking to recruit a Practice Nurse for 18 hours per week to enhance our Practice Nursing Team. We are a welcoming practice in Grimsby aiming to provide high quality care in a supportive environment.
We are looking for a committed, hardworking nurse with the enthusiasm to learn new skills and adapt to changing circumstances. Experience in general practice is preferred with competencies in immunisation and vaccinations, wound care and diabetes (or a minimum of one chronic disease) management. Additional skills of cytology, family planning and management of additional long term conditions will be advantageous.
The successful applicant will take a share of the workload across our modern and well equipped purpose built sites. The role will include chronic disease management, cytology, treating minor illnesses, dressings, family planning, immunisations, vaccinations and health promotion. Main duties of the job
The post holder is responsible for the delivery of practice nursing care to the practice population. They will deliver care within the boundaries
of their role, focusing upon chronic disease management, supporting GPs in
care planning and monitoring of patients at risk of unplanned hospital
admissions, supporting patients to be healthy, monitoring of long-term
conditions, health prevention and screening activities.
You will work collaboratively with the general
practice team to meet the needs of patients, supporting the delivery of policy
and procedures, and providing leadership to other staff as required.
The
following services will be provided:
- Nurse led cytology screening service
- Nurse led management of patients with diabetes
- Nurse-led well Woman service, e.g. health checks
- HRT advice/checks
- Nurse-led Family planning service
- To develop sexual health services in line with local guidance
- Implanon fitting and removal
- Swabs
- Administer travel vaccinations
- Administer childhood immunisations
- Spirometry with reversibility
- Asthma checks
- CHD Checks
- COPD checks
- Diabetes checks
- Epilepsy checks
- ECGs (as part of a consultation or if required urgently by a GP).
- Health education, including diet and lifestyle advice
- Giving telephone advice
- Administering Cryotherapy within competence

Pelham Medical Group's clinical team consists of GPs, Pharamcist, ANP, ACPs, Physician Associate, Practice Nurses, Senior Healthcare Assistants and an MSK First Contact Practitioner.
Our Reception, Administration and Management teams make up the non-clinical side of the practice.

for cervical cytology consultations

mental health needs in accordance with the National Framework (NSF) for Mental

immunisation programmes for both adults and children
- Advise, support and, where appropriate, administer
vaccinations for patients travelling abroad
- Promote and deliver evidence-based care for patients
presenting with aural conditions.
- The post holder is required to provide services to the Practices
housebound patients in their own homes to ensure equitable access to primary
care services.

to undertake the following additional duties in the absence of the Health Care Assistants or as and when
directed to do so by the Nurse Team Leader/GP:
- Venepuncture
- Spirometry
- Removal of sutures and clips
- Hypertension health checks
- Dressings and wound checks
The nurse led
services involve assessment of patients, advice regarding treatment and
therapies and referral to other agencies as appropriate. It is crucial that
you have a good knowledge base in order to manage these patients.
