Job summary
Pen-Y-Cae Surgery is seeking a qualified and enthusiastic Practice Pharmacist to join our multidisciplinary team. This role involves working closely with GPs, nurses, and other healthcare professionals to ensure the safe and effective use of medicines across the practice.
Main duties of the job
- Conduct medication reviews for patients, including those in residential and nursing homes.
- Develop and implement pharmaceutical care plans to optimise prescribing practices.
- Provide clinical supervision and leadership to the medicines management staff.
- Assist in managing and monitoring prescribing budgets.
- Deliver medicine awareness projects and produce patient information materials.
- Support the implementation of rational, cost-effective prescribing in line with local and national objectives.
- They must be able to work independently, using their own initiative, whilst also demonstrating excellent collaboration skills, contributing fully to team projects, quality improvement activities, and wider practice initiatives.
- Development robust systems to manage medicines safely during transitions of care particularly from secondary care settings ensuring a seamless, safe, and effective transfer process.
- An interest in DOAC provision would be desirable
