Job summary

To provide a high quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education.

Main duties of the job

  • To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems
  • Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
  • Provide travel health advice and administer appropriate vaccinations
  • Provide directly and assist GPs with services
  • Act as chaperone when required
  • Undertake health promotion to help educate patients, select, carry out and interpret results of specific tests
  • To maintain accurate patient records and enter onto the computer using agreed Read Codes
  • Ensure accurate completion of all necessary documentation associated with patient health care
  • To collect data and participate in regular Practice audit and research projects
  • To maintain own annual registration with the NMC and adhere to Code of Professional Conduct
  • To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
  • To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice
  • Any other reasonable duties that may be required from time-to-time

We are a GP practice with two sites in Stretton, Warrington and pride ourselves on always endeavouring to help our patients as best we can.

We are looking for a friendly and motivated person to join our team

We have a welcoming and supportive team of clinicians and admin staff.

The ideal candidate should have previous experience of working in a GP practice or similar environment.

Person Specification

Experience

Essential

  • Experience of working within a nursing team
  • Experience of dealing with vulnerable patients
  • Up-to-date training in immunisations
  • Experience of working in a GP practice
  • Health promotion

Desirable

  • Working with Clinical systems preferably Emis.

Qualifications

Essential

  • Current NMC registration
  • Accredited training in chronic disease management
  • Cervical cytology qualification

Desirable

  • Qualified in Phlebotomy
  • ECG Trained
