Care Coordinator (Palliative Care and Dementia)

Our award winning Palliative care team provide pastoral support to patients with life limiting conditions, and those requiring end of life care and treatment. Upholding the culture and ethos of the national Dying Matters campaign, our care coordinators work with patients and their families / carers to encourage meaningful conversations around patients wishes and experiences related to end of life care.

You will also be part of the ongoing development of a Dementia support service identifying patients living with Dementia who may not yet need regular support, but we can agree a future care plan with when their condition means they need more input from medical and community services.

You can expect to have a robust and supportive induction in the Practice and the role. Regular supervision and pastoral support is provided through line management and the Palliative Care Lead.

Main duties of the job

To proactively ensure that eligible patients receive the correct end of life care and are provided with a dedicated point of contact to address any medical or administrative issues promptly.

This role will be predominantly based at our Crookhorn Surgery for 16-20 hours a week.

About us

Portsdown Group Practice covers the whole of Portsmouth from postcodes PO1 to PO7. We operate from 6 surgeries: Cosham Park House (PO6 3BG), Crookhorn Lane (PO7 5XP, Heyward Road (PO4 0DY), Kingston Crescent (PO2 8AL), Paulsgrove (PO6 4HJ), and Somerstown (PO5 4EZ).

We are our own Primary Care Network (PCN). We have Clinical Partners, Salaried GPs and a comprehensive Nursing and extended clinical team overseen by our Clinical Team Manager. The team consists of Nurse / Paramedic Practitioners, Registered Nurses, HCAs, MSK Physiotherapists, Podiatrist, Dietitian, Social Prescribers and Care Co-ordinators. The Pharmacy team consists of Clinical Pharmacists and Pharmacy Technicians.

Job description

Job responsibilities

    • Ensure eligible patients are contacted through appropriate means of communication to ensure good uptake and attendance
    • Monitor clinics and ensure they are appropriately booked / future booked
    • Review patients in line with protocols, ensuring contact is made when appropriate
    • Undertake searches to identify eligible patients
    • Undertake Cancer Care Reviews if required, within specified timeframes
    • Attend Gold Standard Framework (GSF) Meetings
    • Co-ordinating appointments for end of life patients
    • Be the point of contact for end of life patients
    • Liaise with outside agencies in a timely manner
    • Conduct bereavement calls
    • Invite eligible cohorts through appropriate means i.e. text / call / letter on a regular basis
    • Follow up DNA appointments including fast track cancer referrals
    • Maintaining spreadsheets and data for care planning
    • Write (and review) dementia care plans in conjunction with patients and their families / carers
    • Visiting patients at home, when appropriate, to carry out reviews
