Job summary

We have two exciting opportunities to join our respiratory team and be an integral part of developing our FeNO trial. Portsdown PCN has successfully applied for funding from NHS England to evaluate the use of FeNO testing in different cohorts of patients. We will be a beacon site as part of a national roll-out campaign. The post holder is responsible for ensuring the delivery of safe and effective respiratory nursing care to the whole practices respiratory population. To work collaboratively with the whole general practice team to meet the needs of the patients, supporting delivery of policy and procedures and provide direction to colleagues. Main duties of the job

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a respiratory condition, and to work collaboratively with the whole general practice team to meet the needs of the patients. About us

Portsdown Group Practice covers the whole of Portsmouth from postcodes PO1 to PO7. We operate from 6 surgeries: Cosham Park House (PO6 3BG), Crookhorn Lane (PO7 5XP, Heyward Road (PO4 0DY), Kingston Crescent (PO2 8AL), Paulsgrove (PO6 4HJ), and Somerstown (PO5 4EZ). We are our own Primary Care Network (PCN).

We have Clinical Partners, Salaried GPs and a comprehensive Nursing and extended clinical team overseen by our Clinical Team Manager and consisting of Nurse / Paramedic Practitioners, Registered Nurses, HCAs, Physician Associate , MSK Physio Therapists and care co-ordinators. The Pharmacy team consists of Clinical Pharmacists, Pharmacy Technicians and pre-registration trainee Pharmacy Technicians.

The Practice is well established, financially secure and successful. Over the last few years we have earned maximum QOF points.

Main Purpose of the Post:

  • To work autonomously to undertake the routine assessment of patients demonstrating critical thinking and skills in clinical decision-making in the management of patients, providing clinical management in line with national / local guidelines and in liaison with medical colleagues.

  • To provide a high standard of nursing care in adherence to Practice policies, protocols and guidelines at all times, and ensure maintenance of current clinical knowledge in relation to relevant chronic disease along with basic nursing skills

  • The post holder needs to be flexible in line with the present atmosphere for change within the health service delivery. It is essential that the nurse will work within own level of competency and expand her / his role in accordance with practice requirements and own scope of practice.

Duties & Responsibilities of the Post:

  • To be experienced in nurse-led management and to have a high level of knowledge of Respiratory Disease, specifically Asthma and COPD, and to have undertaken educational courses, minimum foundation level but ideally hold the relevant diploma or certificate level (or be willing to undertake these) to ensure effective competency within the role.

  • To be a nurse with specialist knowledge in the delivery of Respiratory services to patients, ensuring that the needs of the patient are paramount and provide liaison between primary and secondary care in relation to area of responsibility.

  • Take responsibility for own development learning and performance including attendance at regular educational updates and participating in clinical supervision and acting as a positive role model

  • Assess, plan, implement and evaluate individual treatment plans for patients with a known respiratory condition within practice guidelines and promote health and well-being , and prevent adverse effects on health and well-being (ie, smoking cessation, diet, exercise) within practice guidelines

  • To be competent in the undertaking of investigatory tests i.e. peak flow, and understand the results of investigations to be able to have input in the diagnostic process and to manage condition.

  • Be aware of acute exacerbations and be able to refer where appropriate.

  • Be competent in inhaler technique, able to assess chose and teach correct use of inhaler devices and spacers as well as being able to teach correct technique when using a peak flow meter.

  • To hold ARTP full certification in spirometry (or be willing to undertake this)

  • To have a good understanding of QOF requirements and support the Practice in its data collection to achieve all relevant targets.

  • Proactively identify and screen patients at risk of developing a long-term condition as appropriate within practice guidelines

  • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of competence. Work with patients in order to support adherence to prescribed treatments

  • The post holder should work within their own level of clinical competency and scope of practice, referring other health care professionals as appropriate.

  • To provide patients and carers with psychological support by discussing relevant intervention and treatment. Providing information on local support networks available.

  • To provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning to ensure competency and compliance.

  • Where the post holder is an independent prescriber; to prescribe safe, effective and appropriate medication as defined by current legislative framework.

  • Be aware of practice-based commissioning and current initiatives to improve existing patient services and be actively involved where appropriate.

  • To work with Partners, Group Business Manager and Clinical Team Manager in identifying issues related to respiratory nursing care and participate in the planning and implementation of changes within area of care and responsibility.

Administrative and Professional Responsibilities:

  • Maintain confidentiality as per NMC The Code for Nurses and Midwives (2018).

  • Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines.

  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

  • Assist with the development of practice protocols.

  • Attend and participate in practice meetings as required.

  • Awareness of statutory protection procedures including referral and recognition of signs and symptoms of child abuse.

  • Awareness of referral mechanisms for mental health issues, family violence, vulnerable adults and addictive behaviours.

  • Actively take part in mandatory and internal / external training as requested.

  • Be aware of government guidelines in relation to confidentiality, clinical information etc.
