Job summary
Due to the expansion of our progressive Practice an
additional vacancy has become available to join our team as a Care Coordinator,
with a focus on cancer and palliative care. Your role will be integral in
ensuring our eligible patients receive appropriate palliative care and support throughout
the year. You will be the point of contact for our end of life patients
and their families / carers.
Whilst the role will have a base site, there will be need and expectation
to work at other sites as appropriate, to suit the needs of the patients as
well as site specific meetings.
Main duties of the job
To proactively
ensure that eligible patients receive the correct end of life care and are
provided with a dedicated point of contact to address any medical or
administrative issues promptly.

Job responsibilities
- Ensure eligible patients are contacted through appropriate means of communication to ensure good uptake and attendance
- Monitor the clinics and ensure they are appropriately booked / future booked
- Report any potential issues with ensuring 100% appointment uptake to the Lead STL
- Searches to identify eligible patients
- Undertake Cancer Care Reviews where required, within specified timeframes
- Attend Gold Standard Framework (GSF) Meetings for each site
- Co-ordinating appointments for end of life patients
- Be the point of contact for end of life patients
- Liaise with outside agencies in a timely manner
- Conduct bereavement visits
- Invite eligible cohorts through appropriate means i.e. text / call / letter on a regular basis
- Follow up DNA appointments
- Assist in basic reception duties on site, as required in cases of exceptional absence
- Assist in general contact centre duties, if required
