Job summary
An opportunity
has arisen for an enterprising Nurse / Paramedic to join our clinical team at
an exciting time for the Practice, in a trainee role. Our Model of Care will
see you working alongside a multi-disciplinary team of experienced GPs, Nurse /
Paramedic Practitioners and Clinical Pharmacists.
You must be able to communicate effectively with a wide range of healthcare professionals and be enthusiastic and committed to making a difference and improve practice through ongoing personal and professional development.
You will be supported to deliver high
standards of care using advanced autonomous clinical skills, offering support
to both our nursing and medical colleagues.
You will be given
agreed dedicated time for academic study during your training and be expected
to manage your time wisely to gain the experience and knowledge to carry out
this role in Primary Care. The successful applicant will have recent evidence
of level 6 / 7 study, and be willing to undertake the demanding Advanced
Practice MSc level 7 programme.
The post
is offered on a 2 Year Training Programme basis.

completion of the course of training, that a minimum of 2 years service with
the Practice is required, otherwise the applicant agrees to reimburse the costs
incurred by providing the course.
Main duties of the job
The post holder will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
The role will be varied and wide-ranging, including telephone triage, face to face same-day consultations and home visits, supported by an experienced team of dedicated clinicians.

A Trainee Nurse / Paramedic
Practitioner at Portsdown Group Practice will be an experienced clinician with
the drive and ambition to develop their skills who, acting within their
professional boundaries with training, clinical supervision and development,
will provide care for patients presenting at the practice from initial history
taking, clinical assessment, diagnosis, to treatment and evaluation of care.
They will demonstrate safe, clinical decision-making and expert care, including
assessment and diagnostic skills, for patients within the general practice.
The post holder will
undertake a 2 year Masters programme in Advanced Clinical Practice to enable
them to work towards independent, autonomous clinical practice as an Advanced
Practitioner during training. The post holder will be allocated a Medical
Supervisor and Advance Practitioner Mentor to ensure successful achievement of
the clinical skills required to fulfil the role of Advanced Practitioner. The
trainee will be identifiable in their new role as an Advanced Clinical
Practitioner in training. As part of the training programme the post holder
will be released for at least 1 day per week (pro rata) for study / placements
required to successfully complete the programme. On completion of the 2 year
programme, s/he will exercise advanced clinical expertise, levels of judgement,
discretion and decision making in clinical care demonstrated through the Trust
advanced competency framework, whilst maintaining a professional portfolio.
S/he will provide advanced clinical skills and advice demonstrating a sound
understanding of the issues related to the identification, assessment,
diagnosis, treatment and management of the patient group. S/he will monitor and
lead improvements to standards of care through, supervision of practice,
clinical audit, evidence based practice, teaching and supporting professional
colleagues and the provision of skilled professional leadership. S/he will lead
the implementation of relevant research into practice contributing to the
further development of the evidence base through research and audit.
