Main Purpose of the Post:
To provide predominantly remote support to the practice for eConsultations and administrative work to ensure high standards of patient care
Duties & Responsibilities of the Post:
Clinical
Use your professional skill and knowledge to provide a high standard of primary and continuing medical care to patients in line with the GMS contract
Issue and authorise (EPS / signature) computer-generated acute and repeat prescriptions in accordance with the Practice / Portsmouth CCG prescribing formulary, whenever this is clinically appropriate. Dealing with prescription queries.
Complete clinically related administrative and non-clinical duties needed for the delivery of the service for
NHS and non-NHS agencies, e.g. solicitors, insurance companies and other third parties
Summarise patient records and read-code patient data
Process and action incoming patient and hospital correspondence.
Undertake remote consultations that are required as a result of patient initiated eConsult
Refer patients to other care providers as appropriate, utilising e-Referrals where required in a timely manner and in line with Practice and CCG guidelines.
Record clear and contemporaneous consultation notes to agreed standards, ensuring appropriate use of clinical system templates and read codes.
Request, perform and / or interpret tests and records, reports and examination information to assist in diagnosis of patients condition. Prescribe or administer treatment, therapy, medication, vaccination and other appropriate medical care to treat or prevent illness, disease or injury. Monitor patients condition and progress and re-evaluate and / or change treatments as necessary.

Other Responsibilities within the Organisation
Be aware of and comply with all relevant practice policies / guidelines, e.g. prescribing, confidentiality, data protection, health and safety
Ensure that the practices complaints and significant events procedure is adhered to at all times.
Training & Development
Fully participate in training / events, meeting all requirements of any courses funded or part-funded by the Practice
Attend in-house meetings, significant event audit meetings and other meetings as deemed necessary
Key Relationships - Mentoring, Supporting and Educating Others
Act as an advocate for patients and colleagues
Promote an open, reflective and learning culture and use reflective practice to analyse incidents and events and to develop high quality care.
Actively support branch and community teams when needed, so that those teams can deliver high standards of care
Develop own professional skills and ensure these are shared with the team and primary healthcare team
Confidentiality:
As per both Government legislation and Practice Policies ensure that all confidentiality, data protection and information governance policies and guidelines are followed and strictly adhered to, reporting any infringements to the Operations Office immediately.
Health & Safety:
The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include:
Using personal security systems within the workplace according to practice guidelines
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
Making effective use of training to update knowledge and skills
Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
Actively reporting of health and safety hazards and infection hazards immediately when recognised
Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role
Undertaking periodic infection control training (minimum annually)

Policies and Procedures - the duties and responsibilities of the post will be undertaken in accordance with the policies, procedures and practices of the Practice, which may be amended from time to time. You are required to be flexible and the practice reserves the right to alter such fixed hours as may be considered necessary to ensure the surgery runs smoothly.
The business operates between the hours of 0700 2100 hours Monday to Friday (0800 1300 Saturday), with possible requirement for some future evening and weekend working as the business develops.
