Beechfield Medical Centre
is a well-established and highly regarded practice based in the heart of the
Lincolnshire market town of Spalding. We operate from modern purpose built
premises and we are now looking to strengthen our team further with the appointment
of A further Paramedic Practitioner as we look to develop and enhance the
services we are able to offer to our 20000 patients, particularly as we
integrate our services for the wider community of a Primary Care Network within
our locality.
This
is an exciting role where proactivity, initiative and a can do approach to
service development will be as important as the professional experience and
range of clinical skills that you can offer the Practice.
Main duties of the job
To assess, diagnose, treat, refer or
signpost patients who attend the surgery with their illnesses. Experience in acute illness management in Primary care setting is desirable.
To undertake home visits, primarily
for acute conditions, as assessed by the Duty Doctor, and within the boundaries
of professional qualifications.
To use your advanced clinical skills
to provide education to patients, promoting self-care and empowering them to
make informed choices about their treatment.
To provide appropriate cover, in
association with other clinicians and agree to take part in extended access
which although is often run by our HCAs and Doctors but still agreement to
take part is required when necessary

Paramedic PRIMARY CARE PRACTITIONER

Paramedic/Practitioner, Partners and Practice Manager.
Main purpose of Post:
To
work as part of the Clinical Team, the post holder will be an experienced
paramedic/extended skills practitioner. Provide care for the patients from
initial history taking, clinical assessment, diagnosis, treatment and
evaluation of care.
They
will demonstrate safe, clinical decision making, acting within their
professional boundaries and qualifications. Provide face to face triage
consultations and visit patients in their homes. They will also work collaboratively
with the Partners and Lead Nurse to effectively and efficiently meet the needs
of the patients.
Clinical Duties
- To undertake independently face to face
consultations and home visits where deemed necessary.
- Assess, diagnose, plan, implement and
evaluate treatment for presenting patients, following practice policy and
prescribing formulary.
- Assess patients needs from a
physiological and psychological perspective.
- Chronic disease management.
- Arrange and interpret test results.
- Refer patients to secondary care or
community services when clinically necessary and appropriate
- Issue appropriate treatments following
nurse/practice formulary and ensure prescriptions are signed by duty
doctor.
- Support patients to adopt health
promotion strategies that promote healthy lifestyles and apply principles
of self-care where appropriate
- To undertake where possible data
collection for QOF during contacts with patients.
- To support and facilitate learning to new
and existing members of the clinical team.
Administration
and Professional responsibilities
- Ensure accurate and legible notes of all consultations and
treatments are entered in patients electronic records according to
practice policy.
- Ensure accurate completion of all necessary documentation
associated with patient health care within the practice.
- Ensure collection and maintenance of statistical information
required for regular ad hoc reports and audit.
- To undertake incident reports where appropriate.
- Undertake risk assessment where applicable.
- Attend and participate in both practice and nursing meetings as
required.
- To undertake research studies independently or in conjunction
with other health care professionals with the support and guidance from
clinical team, with the objective to improve or re-evaluate patient care
and services.
Training
and professional development
- Training and development will be monitored by yearly appraisal
and the formulation of a personal learning plan in conjunction to the
practice learning plan.
- Professional development will be encouraged and supported by the
practice. It is the individuals responsibility to keep their CPD up to
date and ensure they are registered with the NMC/HCPC or appropriate body.
- It is the responsibility of the practitioner to ensure they
develop and maintain a personal learning plan.
- Participate with the education and training of students of all
disciplines and introduction of all members of the practice staff where
appropriate.
- If it is necessary to expand the role to include additional
responsibilities appropriate training will be given.
- To provide additional duties as requested by partners, clinical skills
lead and
practice manager.
Communication
- Good
communication and relationships must be established and maintained with
patients, colleagues and employers at all times.
- Demonstrate sensitive communication styles to ensure patients are
fully informed and consent to treatment
- Communicate effectively with patients and carers, recognising the
need for alternative methods of communication to overcome levels of
understanding and cultural backgrounds
- Work collaboratively with reception and administration staff to
ensure the smooth running of the practice, reporting any problems
encountered to the relevant person/persons.
- There will also be a need to establish and maintain good liaison
with other surgeries, agencies and secondary care.
- Ensure that the practice manager is informed of any matters
arising or problems/potential problems.
