Job summary

To provide leadership and support to the MMP nursing and health care team. They will have direct contact with patients, focus on supervising employees and act as a link between nursing staff and MMP management team.

The position requires dynamic leadership skills to support the nursing team to deliver patient care and the highest level of quality and safety.

The lead nurse will be responsible for reviewing clinical guidelines, risk assessments and policies and protocols as agreed by Chief Nursing Officer and the MMP Medical Directors.

The lead nurse will support the Chief Nursing Officer to oversee performance indicators, and nurse led clinical targets including but not limited to vaccination and screening.

Main duties of the job

To undertake any other duties commensurate with the role, within the bounds of their own competence.

To work across the various MMP sites as required.

In light of national policy and due to the needs of the business it may be necessary for the Partnership to alter the opening hours of the surgeries. This could incorporate different opening hours and weekend working which may effect when you are required to work. The post holder is expected to be flexible and accommodating, following consultation, in terms of any changes to operating times in the future.

About us

Midlands Medical Partnership is a Birmingham super-partnership currently caring for 72,000 Birmingham Cross-City CCG patients across North and South Birmingham operating out of 10 sites. The partnership was established in April 2009 and continues to grow as a single integrated primary care provider, alongside collaborative working with other similar organisations in our region as part of its strategic forward view.

This dynamic partnership with a team of over 200 clinicians and staff is managed by a single executive board, with centralised organisational functions, strong leadership and clinical governance structures. Clinical

Recognise and work within own competence and professional code of conduct as regulated by the NMC Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Perform routine tasks and general practice nurse duties related to patient care in line with the needs of the organisation.

Health promotion for patients with/at risk of a long term condition

Manage and treat long-term conditions, in line with national and local policies and organisational needs. ( Diabetes, Asthma, COPD, Hypertension, CHD)

Prescribe and review medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments

Prioritise health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Implement and participate in vaccination and immunisation programmes for both adults and children including patients travelling abroad

Implement and participate in screening services including but not limited to Cervical Cytology Screening.

Implement and review the application of evidence-based practice in nursing

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required

Any other delegated duties appropriate to the post.

Organisational

Prioritise, organise and manage own and others workload in a manner that maintains and promotes quality.

Support the Chief Nursing Officer to provide leadership, guidance and advice on CQC nursing issues, ensuring that Nurse led services and duties are delivered in line with all regulatory requirements.

Planning and implementation of local guidelines, protocols and standards, and of local projects or initiatives.

Participate in shared learning across the practice and wider organisation and assess the impact of policy implementation on care delivery.

Support with Site Health Checks as and when required and ensure they are completed in accordance with MMP Policies.

Ensure the highest standards of infection control for all nurses and associated teams across the organisation.

Ensure Cold Chain Policy and Procedure is adhered to by all MMP nursing staff and sites.

Ensure that Emergency trolley and Emergency Drug procedures are adhered to by all MMP staff and sites.

Ensure that weekly cleaning schedules are adhered to MMP staff and sites.

Undertake and manage the Clinical Supervision for MMP Nurses, HCAs and HCT team as agreed with Chief Nursing Officer.

Mentor Newly Qualified Nurses, Nurse new to General Practice and Nursing Associates through a comprehensive MMP preceptorship year.

Act as a mentor for General Practice Nurses providing support as and when required.

Act as a mentor HCAs providing support as and when required.

Act as mentor HCTs providing support as and when required.

Participate in student mentorship.

Ensure regular communication within the Lead Nurse Team. Attend Lead Nurse Team meetings.

Act as Deputy Chair for MMP Nursing team meetings.

Support with recruitment new MMP nursing team staff, including scoring of applications and interviewing.

Ensure robust PGDs and PSDs process at all sites as directed by Chief Nursing offer and Clinical Governance team.
