Job summary

The objective of the Digital and Transformation Lead is to provide a central source of leadership and management to aid service transformation for MMP PCNs. The post holder will need to be proactive, have a passion for Primary Care, and have a proven track record in delivering Primary Care improvements.

The remit of the role is broad and will require attention to detail in delivering high-quality day-to-day operations along with providing strategic leadership and management to the service delivery function, ensuring that a high quality and effective service is delivered

About us

Midlands Medical Partnership is a Birmingham super-partnership currently caring for 72,000 Birmingham Cross-City CCG patients across North and South Birmingham operating out of 10 sites. The partnership was established in April 2009 and continues to grow as a single integrated primary care provider, alongside collaborative working with other similar organisations in our region as part of its strategic forward view.

The vision behind the creation of MMP is to improve and develop all that is excellent about traditional GP services local GPs leading and working in local surgeries to provide long-term doctor/patient relationships based on the established values of trust and personalised care, whilst at the same time being part of a larger, more corporate business structure, allowing our practices and patients to benefit from the resources of a larger organisation, enabling us to improve the range and quality of patient services, improve access and patient responsiveness and improve learning across our organisation.

MMPs core purpose is to work within a culture of quality to constantly improve the care and service we give our patients.

This dynamic partnership with a team of over 200 clinicians and staff is managed by a single executive board, with centralised organisational functions, strong leadership and clinical governance structures.

Job responsibilities

Strategic Planning

To lead and develop a strategic plan that includes a robust digital infrastructure taking into consideration the populations health needs and health inequalitie

To clarify objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirements

Lead on the development and implementation of integration with other services and stakeholders including Integrated Neighbourhood Teams (INTs) in MMP PCN Localities

Lead the Integrated Neighbourhoods Teams Programme in MMP PCN Localities, including the development of strategy and priorities

Promote the engagement of all PCN practices and act as an ambassador for the network.

To proactively build relationships with system leaders to help influence proactive and new models of care

To use business intelligence tools to aid data-driven decisions

To lead the development and implementation of Digital Transformation bids

Operations

To provide oversight and accountability to the delivery of PCN contracts and specifications

Coordinate training and development of PCN staff

Identify and record risks and issues and develop contingency plans with Network Leads

Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented

To drive issues through to resolution, acting as a point of expertise

To implement network-based projects

Finance and Governance

To create and maintain accurate documentation in relation to finances, Standard Operating Procedures, Contracts, Contacts and decision making which is accessible centrally

To support the collation of financial information

To monitor and check the accuracy of the submissions for the DES and contractual requirements

Compiling and presenting financial reports

To ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness.

To ensure the PCN Maximises its business intelligence tools

Develops a continuous improvement approach to digital accessibility for our patients and workforce

Creates data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience

To foster a culture of collaboration and promote learning to drive continuous improvement

To work as a flexible member of the PCN, providing support to other team members when necessary and working the PCN management team

Develop and deliver a communication plan using a wide variety of channels

To take an active role in the development and embedding of the PCN's culture, values and reputation

To represent the network at external meetings

HR and Workforce Planning

Support recruitment, induction and training of new PCN staff

Support recruitment which is aligned to a workforce plan

To facilitate progression for the PCN staff to promote retention
