Job Summary:
We are
looking for an enthusiastic and proactive Clinical Pharmacist to enhance
our clinical team. You will work closely with GPs, nurses and other health care
professionals to improve medicines optimisation, provide structured medication
reviews, and support safer, evidence-based prescribing.
This is an
exciting opportunity for a pharmacist who is passionate about improving patient

Main duties of the job
Summary of Duties
Deliver structured medication reviews, particularly for patients with long-term conditions or on multiple medications.

Ensure safe, effective, and cost-efficient prescribing, including monitoring high-risk medicines and managing repeat prescriptions.

Conduct patient consultations (in person or remotely) and support deprescribing where appropriate.

If an independent prescriber, manage minor ailments and chronic conditions within scope of practice.

Lead on medicines optimisation, audits, QOF, and prescribing incentive schemes.

Collaborate with community pharmacies, care homes, and NHS teams to ensure seamless care.

Provide education and training to staff, keep up to date with guidance, and mentor pharmacy team members when needed.

Clinical Care:
- Conduct structured
medication reviews, particularly for patients with long-term conditions or
polypharmacy.
- Support safe and effective
prescribing, including monitoring high-risk medicines and managing repeat
prescriptions.
- Provide patient
consultations (face-to-face, telephone, or video) on medicines-related
queries.
- Work with clinicians to
reduce medication-related harm and promote deprescribing where
appropriate.
Independent Prescribing (if qualified):
- Prescribe within clinical
competence and monitor treatment effectiveness.
- Manage minor conditions or
chronic disease reviews under agreed protocols.
Medicines Optimisation & Practice Support:
- Lead on audits, QOF, IIF,
and local prescribing incentive schemes.
- Liaise with community
pharmacies, care homes, and local NHS teams.
- Advise on cost-effective
prescribing in line with local formulary and NICE guidance.
- Promote adherence and
educate patients on safe medication use.
Education & Development:
- Provide training or updates
for practice staff on medicines and prescribing topics.
- Stay up to date with
clinical guidance and attend PCN/CPPE pharmacist events.
- Support the mentoring of pharmacy
technicians, if appropriate.
