Job Description
JOB TITLE: Practice
Nurse
REPORTS TO: GP Partners/Nurse (Clinically)
Practice Manager (Administratively)
HOURS: Part Time (25 Hrs) can be negotiated
Job Summary:
The
post holder is responsible for the delivery of general practice nursing to the
whole practice population. The focus of
the role is both the delivery of evidence-based practice for patients
presenting with a long-term condition, and the provision of preventative health
care to the practice population. As an
autonomous practitioner the nurse is responsible for the management of
patients. They will work collaboratively
within the general practice team to meet the needs of patients, support the
delivery of policy and procedures, and provide nurse leadership as required. The post holder will be required to work at
any one of the 2 sites.
Job Responsibilities:
Provide
nursing treatments to patients in participation with general practitioners or
independently to agreed protocols.
Provide
general and specific health screenings to the Practice patients (within agreed
protocols) with referral to general practitioners as necessary.
Advise
patients on general health care and minor ailments with referral to GPs as
necessary.
Nursing procedures
to include, but not limited to
Assessment, Screening & Treatment Services
Blood pressure checks
Urinalysis
Dietary advice
Health screening
Cervical cytology
Phlebotomy
Travel vaccinations and advice
Contraception advice/checks
Routine immunisations and vaccinations
Baby immunisations
Diagnose and manage chronic conditions
First registrations checks/health surveillance/general health advice
Conduct home visits
Take lead for infection control monitoring and compliance. Deliver basic infection control training to all members of staff.
You
will be fully conversant in QOF QOEST, read coding and uphold strategies to provide a
holistic combined healthcare for the patient and demonstrate a willingness to
participate in research/audit within the practice team.
You
will be aware of DES/LES/IIF policies and apply and produce, alongside the Practice
Manager, protocols that reflect the guidelines that are workable in the
practice.
Participate
in the education of students of all disciplines.
To
adhere to strict Infection Control in line with guidelines and policies.
Supplies
and equipment Treatment room and other areas
Pathological specimens and investigatory procedures
Following
appropriate training, undertake the collection, storage and despatch of
pathological specimens including intravenous blood samples, swabs, smears etc
and ensure the cleanliness of any equipment so used. Perform any investigatory procedures
requested by the GPs.
Administrative and professional responsibilities
Participate
in the administrative and professional responsibilities of the Practice team.
Ensure
accurate notes of all consultations and treatments are recorded in the
patients notes on the clinical computer system as appropriate.
Ensure
accurate completion of all necessary documentation associated with patient health
care and registration with the Practice.
Ensure
appropriate items of service claims are made accurately, reporting any problems
to the Practice Manager
Ensure
collection and maintenance of statistical information required for regular and
ad hoc reports.
Attend
and participate in Practice meetings as required.
Maintain a notice
board in the waiting area designated for patient health care education.
Maintenance of
equipment and stock relating to patient care
Teamwork
- All employees are

undertaking the work of absent colleagues or assisting others where the
workload is particularly high.
Accordingly, there may be a requirement, from time to time, to
undertake other duties.
Liaison
- Maintain
effective liaison with other agencies and staff concerned with patient
care and with all other disciplines within the Practice, with appropriate
regard to confidentiality.
Meetings
- It
will be necessary to attend and contribute to various Practice meetings as
requested. The only reason for not
attending will be annual, study or sick leave
Professional
development
- Maintain
continued education by attendance at courses and study days as deemed
useful or necessary for professional development.
Confidentiality:
- In the course
of seeking treatment, patients entrust us with, or allow us to gather,
sensitive information in relation to their health and other matters. They do so in confidence and have the
right to expect that staff will respect their privacy and act
appropriately
- In the
performance of the duties outlined in this Job Description, the
post-holder may have access to confidential information relating to
patients and their carers, Practice staff and other healthcare
workers. They may also have access
to information relating to the Practice as a business organisation.
All such information from any source is to be
regarded as strictly confidential
- Information
relating to patients, carers, colleagues, other healthcare workers or the
business of the Practice may only be divulged to authorised persons in
accordance with the Practice policies and procedures relating to
confidentiality and the protection of personal and sensitive data
- The Data
Protection Act 1998 and Caldicott Principals must be adhered to
Health & Safety:
The post-holder will assist in promoting and
maintaining their own and others health, safety and security as defined in the
Practice Health & Safety Policy, to include:
- Using personal
security systems within the workplace according to Practice guidelines
- Identifying the
risks involved in work activities and undertaking such activities in a way
that manages those risks
- Making
effective use of training to update knowledge and skills
- Using
appropriate infection control procedures, maintaining work areas in a tidy
and safe way and free from hazards
- Reporting
potential risks identified

The post holder must keep up to date with the requirements of the

mandatory training and follow the Practice Policies and Procedures to ensure

Safeguarding:
All employees have a duty for safeguarding and promoting the welfare of
children and adults at risk. The post
holder must be familiar with, and adhere to, the Practices Safeguarding Policy
and the process for raising concerns about the welfare of anyone with whom they
have contact. The post holder must also
attend or undertake the appropriate level of Safeguarding Children and Adult
training.

The
post-holder will participate in any training programme implemented by the
Practice as part of this employment, such training to include:

work
