Job summary

The Nunhead Surgery started in the 80s when a previous Partner took over the lists of 2 retiring GPs.We built new premises under the cost rent scheme in July 1993. We have a relatively small catchment area with many patients who have been registered with us since the onset. We are patient focused and always try to offer the best possible patient care within our control. The whole team is passionate about making a difference for our patients and improving local population health and living.

Our list has grown significantly over the years and has continued to grow. We regularly review our catchment area, ensuring we are inclusive and offering services to our immediate population.

We also offer many additional services and are always keen to pilot any new service that could benefit our local population. We work closely with our federation and are active within our Primary Care Network. We try to host as many services as possible as we believe having the services in-house, enables our patients to feel comfortable accessing the range of professionals available. We currently offer appointments with: Mental Health Practitioner, Wellbeing Coach, Social Prescriber, First Contact Physio and a Care Coordinator. In addition to this we have two PCN pharmacists as well as an employed pharmacists supporting the clinical team and patients with medication queries, compliance and monitoring.

Main duties of the job

We are seeking an experienced and motivated Advanced Nurse Practitioner to join our friendly and supportive GP practice for 2 days per week.

This is an excellent opportunity to work within a well-established primary care team, delivering high-quality, patient-centred care in a collaborative environment.

About the Role

The successful candidate will:

  • Provide autonomous clinical assessment, diagnosis, and treatment of acute and chronic conditions

  • Manage same-day presentations and minor illness clinics

  • Prescribe independently (V300 qualification essential)

  • Work collaboratively with GPs, nursing staff, and the wider multidisciplinary team

  • Contribute to practice development and quality improvement

About us

We are located in Nunhead which is an up-and-coming residential neighbourhood with a good community feel. We have been in our existing building for 30 years and feel like an important part of our community.

We have 4 partners 3 GPs and a Practice Manager. Some of our staff has been with us for many years, with quite a few through to retirement. Prioritising staff welfare is a very important part of our values and mission, as looking after our own enables us to be stronger to look after our patients.

Job Title: Personal Medical Services - Advanced Nurse Practitioner

Accountable to:Partners

Job Summary:

As part of The Nunhead Surgery, you will provide NHS primary health care to patients registered with the practice. You are expected to participate in practice activities including meetings and audits, as well as taking on a role in the development of the practice as outlined in your contract.

Clinical Responsibilities:

  • Work autonomously within professional scope of practice.

  • Assess, diagnose, treat, and prescribe for acute and chronic conditions.

  • Refer appropriately and escalate concerns when necessary.

  • Establishing appropriate systems to manage common chronic medical conditions, e.g. asthma and diabetes.

  • Providing appropriate preventative health care and advice within the context of primary health care.

  • Maintaining patients clinical records, to include correct problem titles, coding of appropriate values (e.g. BP), coding of appropriate values associated with chronic LTCs in line with QoF and correct use of templates.

  • Maintaining patients confidentiality.

  • Providing free certification in line with schedule 9 of the GMS Regulations (1992).

Other Responsibilities:

Attending or chairing primary health care meetings and other meetings as may be necessary.

Auditing practice activities as required.

Following practice and local guidelines and protocols

Taking part in the development and implementation of Practice Development Plan.

Clinically related administrative and non-clinical duties needed for the delivery of primary care services.

Engaging in appropriate educational activity in order to maintain clinical and managerial competence and performance.

Condition of Appointment:

  • Current NMC registration

  • Advanced Nurse Practitioner qualification

  • Independent Prescriber (V300) qualification

  • Ongoing compliance with NMC revalidation requirements

Proof of qualifications must be provided prior to commencement.

  • Enhanced DBS clearance required.

  • Evidence of right to work in the UK required.

  • The post holder must maintain appropriate professional indemnity cover in addition to any cover provided by the practice or NHS scheme.

Continuing Professional Development (CPD)

  • The ANP is responsible for maintaining CPD in line with NMC requirements.

  • Study leave and funding arrangements will be in accordance with practice policy.

Appraisal

  • Annual appraisal in line with practice governance processes.

  • Participation in audit, SEA, and quality improvement activities required.

Confidentiality & Data Protection

  • Must comply with GDPR and practice confidentiality policies.
