Job summary

We are looking for an innovative, enthusiastic, qualified nurse, preferably with Practice Nurse experience to join our friendly team at The Westbank Practice.

The successful candidate should be able to work autonomously within our team providing care to a varied range of patient's needs including; Chronic Disease management (an interest in diabetes would be preferred but not essential), travel, child immunisations, wound management, contraception, cervical screening, ear care and general treatment room duties. There may be a requirement to work occasional weekends on a rota basis. 

Practice Nurse experience would be ideal, but training may be considered for the right candidate.

Main duties of the job

To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, (a background in diabetes is desireable but not essential) cervical screening, as well as supporting the management team in the reviewing of clinical policy and procedure.

About us

The Westbank Practice is a busy growing practice providing care and support for approximately 9,000 patients across our Exminster and Starcross surgeries. We are a teaching practice with Registrars, F2s and Medical Students on rotation.

Our clinical team consists of 4 GP partners, 3 salaried GPs, one Paramedic, four Practice Nurses, two Health care assistants and one phlebotomist. Beautifully located on the outskirts of Exeter, we are looking for a dynamic nurse to join our team and meet the needs of our growing population.

Job responsibilities

The following are the core responsibilities of the practice nurse (this list is not exhaustive, and some roles and responsibilities may not be dealt with by all members of the team). There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

· To assist as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the practice manager

· To produce the IPC annual statement in conjunction with the lead nurse.

· To develop, implement and embed health promotion and well-being programmes

· To implement and evaluate individual treatment plans for chronic disease patients

· To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients’ health

· To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

· To provide wound care (ulcer/Doppler etc.) to patients

· To provide travel medicine services (post covid)

· To request pathology services as necessary

· To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

· To maintain accurate clinical records in conjunction with extant legislation

· To ensure read codes are used effectively

· To chaperone patients where necessary

· To assist GPs with minor surgery when required

· To prioritise health issues and intervene appropriately

· To support the team in dealing with clinical emergencies

· To recognise, assess and refer patients presenting with mental health needs

· To implement vaccination programmes for adults and children

· To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

· To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

· To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

· To support the clinical team with all safeguarding matters in accordance with local and national policies

· To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

· To deliver opportunistic health promotion where appropriate
