Job summary

We offer an exciting opportunity to join the Outer Exeter Primary Care Network (PCN) and shape the health coach role for the delivery of outstanding primary care in the network area. We are looking for an innovative, motivated health and wellness coach who is looking to join our PCN. You will be employed by and mainly based at the Westbank Practice but you will also work with the five practices of the Outer Exeter PCN Westbank Practice, Ide Lane Surgery, Topsham Surgery, Pinhoe Surgery, and Cranbrook Surgery.

Main duties of the job

Your responsibilities will include delivering group and one-to-one sessions on healthy eating, physical activity and health and wellbeing to meet with personalised care plans; engaging with patients to support them in taking an active role in their health; providing advice and support for both patients and colleagues. You will be required to use clinical systems and record baseline and outcome data to present to your supervisor or the PCN.The right candidate will be educated to GCSE or equivalent and have undergone training in motivational coaching, behavioural change and goal setting. The candidate may also hold and NVQ level 3 or other relevant first level professional qualification or be working towards this. You should have excellent communication and motivational skills, an empathetic style and good organisational skills with the ability to work autonomously.

About us

The post holder will be employed by the lead practice (Westbank Practice) on behalf of the network and will be subject to the employment terms and conditions as specified by the lead practice. Their primary contact for all HR concerns will be the lead practices Practice Manager. The role will be the same across the networks practices and will be defined by this role description as agreed by the PCN Executive Board. Day to day clinical supervision will be provided by the GP pharmacy leads in each of the practices.

As this is a new role and training courses are yet to be specifically designed for this. Suitable applicants may be someone with experience in working with motivating and coaching people on heathy behaviours such as personal training, exercise, life coaching, nutrition, mind-body practices, mindfulness, Psychotherapy and other similar disciplines.

The post holder is a Health and Well-Being Coach who acts within their professional boundaries to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing.

The role will be to increase patient ability to access and utilise community support whilst providing access to self-management education, peer support and social prescribing.

Health and wellbeing coaches take an approach that considers the whole person in

addressing existing issues and encourages proactive prevention of new illnesses.

The approach will be to appreciate patients circumstances in a non-judgemental manner. This will result in the need for positive communication and negotiation skills that will support the patients personal choices.

The role will be an integral part of the general practice team working in each practice and across the PCN.

You will be employed by and mainly based at the Westbank Practice in Starcross & Exminster but you will also work with the five practices of the Outer Exeter PCN Westbank Practice, Ide Lane Surgery, Topsham Surgery, Pinhoe Practice, and Cranbrook surgery.

You will report to the Clinical Director of the PCN or their nominees who will be responsible for supporting your ongoing professional development.

Be willing after the initial period of 6 months to extend your coaching services to any practice in the PCN and be willing to travel to these practices to deliver health coaching & wellbeing services.

Primary responsibilities

The following are the core responsibilities of the PCN health and well-being coach. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

1. To work as part of a multi-disciplinary team in a patient facing role to assess and treat patients using expert knowledge

2. To be responsible for the assessment of new patients with subsequent production and completion of individual care plans

3. To coach and motivate patients through multiple sessions (usually up to 6 sessions) to identify their needs set goals, and support them to implement their personalised health and care plan.

4. To provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in manging their own health and well-being, to live independently and to improve their health outcomes through:

a. Providing interventions such as self-management, education and peer support

b. Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual

c. Working with the social prescribing service to connect them to community-based activities which support their health and well-being

5. To be aware of limitations and, as required, be responsible for the referral of patients who require the intervention of other healthcare professionals

6. To work with patients and to understand their level of knowledge, skills and confidence when engaging with their health and well-being

7. To provide support to local community groups and work with other health, social care and voluntary sector providers to support patients health and well-being holistically.

8. To identify risk factors and categorise patients into appropriate risk banding

9. To facilitate groups of patients with group consultations to assist patients to work with others to support their own goals

10. To be responsible for the day-to-day planning of personal workloads

11. To follow departmental policies procedures and guidelines

12. To liaise with the practices and, where practicable, to standardise processes across the PCN

13. To receive referrals and directed patients from triage services and other clinicians (these will usually be patients with low Patient Activation Measurement Scores)

14. To conduct coaching clinics which may be virtual or in person with an allocated patient list.

15. To conduct health and wellbeing classes online or in person available to all patients of the PCN according to your area of expertise.

16. To improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards and clinical and prescribing guidance

17. To develop yourself and the role through participation in clinical supervision, training and service redesign activities. This includes attendance of approved training courses in Health Coaching by the PCN and clinical supervision.

18. To contribute to a patient safety culture through reporting and investigation of incidents and undertaking proactive measures to improve patient safety

19. To actively signpost patients to the correct healthcare professional

20. To maintain accurate clinical records of all patient consultations and related work

21. To promote health coaching and social prescribing amongst the PCN and networked practices and to support colleagues to improve their skills and understanding of personalised care

22. To use the clinical system to record clinical data at baseline and at defined follow-up intervals to record outcome data for presentation to the clinical team and PCN.

23. To review the latest guidance ensuring the practice conforms to NICE, CQC etc.

24. To support in the delivery of enhanced services and other service requirements on behalf of the PCN

25. To participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events

26. To undertake all mandatory training and induction programmes

27. To contribute to and embrace the spectrum of clinical governance

28. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed

29. To contribute to public health campaigns (e.g. flu clinics) through advice or direct care

30. To maintain a clean, tidy, effective working area at all times

Secondary responsibilities

In addition to the primary responsibilities, the health and well-being coach may be requested to:

a. Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives

b. Undertake any tasks consistent with the level of the post and the scope of the role ensuring that work is delivered in a timely and effective manner.
