Job summary

Nexus PCN has an exciting opportunity for a PCN Manager / Digital Transformation Lead to support the Network in enhancing its operations by delivering results, fostering constructive communication, and promptly reporting progress to ensure stakeholders are kept informed and objectives are met. Moreover, the role entails overseeing the management of Nexus Primary Care Network in alignment with its scope, budget, and operational necessities.

Main duties of the job

The role will aim to facilitate continual enhancement of administrative and clinical services, demand proficiency in business / service enhancement and expertise in driving transformative changes. A comprehensive comprehension of general practice systems and procedures is imperative.

The PCN Manager and Digital Transformation Lead will be a vital member of the PCN leadership team, working as part of the Management Committee (Clinical Director, Lead Practice Manager and PCN Support Manager) and collaborating closely with the Nexus PCN Board. This role will serve as a knowledgeable focal point, ensuring the successful fulfilment of all governance obligations, implementing national and local policies effectively, and assisting practices in enhancing outcomes for the local community. Moreover, they will interact with counterparts in other PCNs.

The post holder will collaborate with clinical and operational teams within our Primary Care Network (PCN) to craft a guiding vision for service enhancements. They will seek out opportunities for substantial and positive transformation, offering motivation, resources, and tools to implement changes collaboratively. It is imperative that this vision aligns with the strategy of the local Integrated Care System (ICS).

About us

Nexus PCN is located in the centre of the vibrant city of Exeter in Devon and has a population of around 42,000. There are 5 surgeries within the PCN, within close proximity of each other, with excellent inter-personal relations, a good support network and a history of collaborative working. The practices are Mount Pleasant Health Centre, The Heavitree Practice, South Lawn Medical Centre, Isca Medical Practice and Hill Barton Surgery. Our population is diverse, with varying levels of socio-economic status, young families, elderly patients and university students all of whom make providing care very interesting.

Operational and Organisational Matters

  • Undertake a wide range of day-to-day functions and project management to support Nexus PCN and the team to deliver ongoing improvement to services.
  • Assist with planning, development, and implementation of changes necessary to meet new demands on the Network from internal and external sources.
  • Ensuring effective communications within the PCN and with its key stakeholders.

  • Organising and minuting functional team and managing committee meetings.

Project Management

  • Leading on developing, bidding and operational management of new projects for the Network.
