We have an exciting opportunity for
an experienced OT to join our established OT team part time (approx. 22.5 hours per week) for 12 months starting in January
2025. We would be willing to support secondment opportunities in line with your
current employers agreement.
Our OT team supports patients with complex health needs both in their own homes, and within care homes. We provide holistic assessments with a focus on reducing risks, promoting independence and encouraging self-management of long-term health conditions.
This role will largely be supporting our OT home visit service across the 5 GP practices within the NEXUS Primary Care Network (PCN).

The post holder would be an NHS employee, employed by Mount Pleasant Health Centre on behalf of Nexus.

Main duties of the job
The post holder will work alongside two
experienced band 7 equivalent OTs to triage incoming referrals from staff
across the five practices. The post holder will further complete holistic comprehensive
geriatric assessments with patients at home, identify occupational needs,
providing OT advice and treatment and linking with other services to ensure the
persons functional, health and social care needs are being met.
The post holder will also support
with the running of our Occupational Therapy led Cognition Clinic to
undertake initial memory screening assessments, providing signposting and
advice, and making onward memory clinic referrals as required.
The post holder may also at times be
supporting with the implementation of the Enhanced Health in Care Homes
Framework. They will work closely with the PCN multi-disciplinary team to
improve the quality of care provided in the care homes aligned to PCN GPs. This
will be through provision of excellent OT assessment, advice and intervention by advocating for care home residents and by providing guidance to care home
staff where needed.

Nexus PCN is located in the centre of the vibrant city of Exeter in Devon and has a population of around 42,000. There are 5 surgeries within the PCN, within close proximity of each other, with excellent inter-personal relations, a good support network and a history of collaborative working. The practices are Mount Pleasant Health Centre, The Heavitree Practice, South Lawn Medical Centre, Isca Medical Practice and Hill Barton Surgery. Our population is diverse, with varying levels of socio-economic status, young families, elderly patients and university students all of whom make providing care very interesting.

- Degree or graduate diploma in Occupational Therapy
- Post graduate training Evidence of understanding the statutory framework within which Occupational therapy is provided (detailed knowledge of Occupational Therapy legislation, guidelines and procedures)
- Health and Care Professions Council Registered (HCPC) Registration with The Royal College of Occupational Therapists
Desirable
- Evidence of recent training as a commitment to personal and professional development.
- Membership of relevant Special Interest Groups
