The pharmacist is a key role in delivering the medicines management agenda, focusing on clinically effective, safe and cost-effective prescribing throughout the practice.

The role involves patient-facing and back-office functions and offers flexibility in how this mix is structured. With a small team of clerks and technicians to inspire and supervise, there are some line management responsibilities. As a senior role within the practice, the pharmacist plays an active part in service development and quality improvement, working closely with the partners and management team.

Full time preferred, but for part-time considered for the right candidate.

Main duties of the job

This is a senior role and the successful candidate will be able to advise, encourage and inspire members of the team.

The purpose of the post is to ensure that medicines management processes within the practice are optimised to achieve clinically effective, safe and cost-effective prescribing. Patient-facing tasks include SMRs and chronic disease management subject to qualifications and skills.

You must be a qualified pharmacist registered with the GPC. Previous experience of working in GP practice preferred not essential. The post is intended for an independent prescriber and you should have or to be working towards this qualification.

You must be confident in working independently, able to prioritise workload for yourself and for junior staff and work well as part of a team. Excellent communication skills and an eye for detail, along with honesty, integrity, loyalty and commitment are attributes valued for this role.

MAIN PURPOSE OF THE POST

  • Ensure that medicines management processes within the practice are optimised to achieve clinically effective, safe, cost-effective patient-centred prescribing
  • Undertake patient consultations regarding their medication and long-term conditions
  • Work closely with the practice to deliver the locality-wide priorities with regards to medication.

KRA 1: REPEAT PRESCRIBING AND MEDICINES MANAGEMENT

1:1 Manage addition/change of medication from hospital discharge/letters.

1:2 Deal with queries regarding medication from pharmacies and other healthcare professionals.

1:3 Offer holistic approach to the use and understanding of medicines by patients that includes step up and step down management required in long term conditions or possibly end of life.

1:4 Review, update and implement a practice Repeat Prescribing SOP.

1:5 Provide support for patients with questions, queries and concerns about their medications by telephone or in person.

1:6 Perform medication reviews with the patients ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with the medicine taking.

1:7 Ensure every patient contact is recorded contemporaneously and accurately on the patientselectronic medical record using appropriate encounter details, SNOMED and/or templates.

1:8 As an independent prescriber, prescribe safely, cost effectively and appropriately within current legislative frameworks for both acute and long term conditions within scope of practice, reviewing its effectiveness and appropriateness to each individual patient.

KRA 2: TEAMWORKING

2:1 Promote teamwork and consider other peoples roles and responsibilities.

2:2 Lead regular prescribing meetings, providing data and reports in liaison with the practice business manager

2:2 Attend and take an active part in nurse meetings and other meetings as required.

2:3 Provide training on medication prescribing and process to work colleagues and new members of the practice team.

KRA 3: MEDICINES AND QUALITY IMPROVEMENT

3:1 Make recommendations and implement changes to maximise cost-effectiveness.

3:2 Implement CCG medicines management initiatives and position statements

3:3 Develop and manage new services that are built around new medicines or NICE guidance, where new medicine/recommendations allow the development of a new care pathway.

3:4 Assist in monitoring costs and budgets.

3:5 Provide audit data for the practice and the CCG.

3:6 Work with the network on the network-wide projects.

KRA 4: LINE MANAGEMENT OF PHARMACY TECHNICIAN AND PRESCRIPTIONCLERKS

4:1 Liaise closely with the practice manager regarding requests for annual leave,sick leave, time owed, performance issues or any other matters concerning staff.

4:2 Distribute tasks to pharmacy technician ensuring this is completed and returned by the deadline.

4:3 Ensure all staff are kept fully aware of all SOPS and policies, and changes to them in a timely fashion.

4:4 Schedule training sessions as per instruction from the management team.

4:5 Meet on a regular basis with the Practice Manager/Business Manager.

4:6 Take part in the appraisal process for the pharmacy technician and prescription clerks.

4:7 Supervise the work of the Network Pharmacy technician in relation to St Pauls work

KRA 5: LONG TERM CONDITION MANAGEMENT

5:1 Undertake long term conditions management including regular monitoring andannual review for patients with those long-term conditions for which you haveappropriate training and experience.

5:2 Keep up-to-date with developments and evidence-based guidelines with regardto long term conditions management, and feed these back to the team.

5:3 Review your agreed long term conditions management protocols on an annualbasis.

5:4 Ensure all long term conditions reviews are entered onto the computer using theappropriate EMIS template.

KRA 6: MISCELLANEOUS

6:1 Work with the partners and managers to ensure the practice is compliant with CQC standards in respect of medications and prescribing systems.

6:2 Responsibility for vaccine, drugs and appliances ordering, stock control and claims with the help of the practice pharmacy technician.

6.3 Be able to take part in the vaccination programmes.

6:4 Plan and organise own workload, including audit and project work.

6:5 Contribute to practice procedures for evaluating, developing, introducing and improving patient services.

6:6 Ensure that professional registration, indemnity insurance, mandatory and role-specific skills are kept up-to-date.

6:7 Maintain the highest standards of personal cleanliness and infection control.

6:8 Take part in the practice annual performance appraisal system.
