Job summary

An exciting opportunity has arisen for a part time Practice Nurse role at Helmsley Medical Centre and Terrington Surgery, due to a planned retirement. This is open to-

Experienced practice nurses, new or recently qualified nurses or nurses who have not worked in general practice before but are considering doing so.

We are able to enrol a new to General Practice Nurse into the vocational training scheme which will allow for a year of supported teaching and learning alongside working in the practice.

We would welcome applicants from community nursing or secondary care settings.

This is an exciting development opportunity for an enthusiastic and committed Nurse to join our existing team.

As a Practice Nurse you will be responsible for the delivery of nursing services to the practice population. Supported by Senior Nurses the post holder will deliver high quality nursing care within their scope of competence, focusing on supporting patients to manage their long-term conditions, address health issues and promote health and well being by delivering education and screening activities.

Main duties of the job

The working hours are 17 hours per week with rotas across both Helmsley Medical Centre and Terrington Surgery to be agreed.

Main Duties and responsibilities

Meeting patients nursing care needs as identified, this will usually be in the surgery but occasional home visits may also be required. Assess, plan, provide and evaluate nursing care to meet those care needs in the practice population

Work collaboratively with the multi-disciplinary team to meet the needs of patients and play an integral part in delivering the key performance indicators for Helmsley and Terrington Practices.

Provide general nursing care, including wound management, and chronic disease management, vaccinations and immunisations of babies, children and adults.

Provide women's health services including cervical screening and contraception

Conduct health assessments and screenings.

Support patients with long-term conditions, providing education and self-management advice.

Participate in health promotion activities and initiatives.

Maintain accurate and up-to-date patient records.

Job responsibilities

Primary key responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

1. Be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager

2. Support in the publication of the IPC annual statement in conjunction with the practice manager

3. Develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

4. Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

5. Develop, implement and embed health promotion and wellbeing programmes

6. Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

7. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

8. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

9. As required, to diagnose and manage acute and chronic conditions

10. Provide wound care (ulcer/Doppler, etc.) to patients

11. Review patient medications to enhance compliance

12. Provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

13. Co-ordinate the provision of travel medicine services

14. Request pathology services and process pathology results as required

15. Oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

16. Maintain accurate clinical records in conjunction with extant legislation

17. Ensure the appropriate maintenance and use of chronic disease registers

18. Participate in well woman and well man clinics

19. Ensure compliance with the practice chaperone policy

20. Assist GPs with minor surgery when required

21. Prioritise health issues and intervene appropriately

22. Support the team in dealing with clinical emergencies

23. Recognise, assess and refer patients presenting with mental health needs

24. Implement vaccination programmes for adults and children

25. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

26. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

27. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

28. Support the clinical team with all safeguarding matters, in accordance with local and national policies

29. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

30. Deliver opportunistic health promotion where appropriate

Secondary key responsibilities
