Job summary

The post holder will demonstrate: critical thinking in the clinical decision-making process, work collaboratively with the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide leadership to the extended primary care team.

An understanding of SystmOne and Accurx would be advantageous but not essential as training can be provided.

Main duties of the job

Clinical Practice

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.

Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example trainee advanced nurse practitioners and the personalised care roles.

We are a busy GP Practice, located in North Yorkshire.Our Patient list size is of around 1700 patients currently. We have a team of 3 GPs, 3 Nurses, 1HCA and 4 admin.

Job responsibilities

Helmsley Medical Centre is located in the small, picturesque market town of Helmsley in North Yorkshire. With a patient population of approximately 3,600, we are a small family-orientated rural practice. Run by two partners, across Helmsley Surgery and Terrington Surgery we have a team of salaried GPs, practice nurses (including a prescribing nurse practitioner) and HCAs as well as a full complement of support staff. We are also an active member of the award winning SHaR PCN and benefit from, not only a PCN run urgent care service but also first contact physios, mental health workers, clinicalpharmacists, SPLWs, care co-ordinators and OTs. As a training practice for Hull and York Medical School, there is the opportunity to teach medical students, and we have recently been nominated for best practice and best tutor awards from our most recent cohort of students. There are CPD opportunities, particularly if you wish to develop a personal interest. We are also a dispensing practice. In addition to clinically diagnosing and treating patients with both acute and long-term conditions, you will be required to undertake indirect clinical tasks such as prescribing, referrals, and managing online consultations. Non-clinical responsibilities include attending meetings, completing paperwork, and engaging in professional development. We are high QOF achievers and were rated good in the last CQC inspection. An understanding of SystmOne and Accurx would be advantageous but not essential as training can be provided.
