Job summary
Working independently via telephone, through face to face consultations and providing a home visiting service, the practitioner will be accountable for delivering high quality, safe, clinical decision-making and expert patient care. Acting within professional boundaries, duties will cover all of the patient journey, including: history taking; examination; holistic clinical assessment; diagnosis; investigation; treatment; supply of medication via PGDs or utilising a prescribing qualification; valuation of patient care and safe handover and communication. An integral member of wider multidisciplinary teams, the practitioner will help formulate and work to advance care plans, together with members of the primary health care team and attached health, social and community services, including paramedics and secondary care colleagues.

- Professional registration (RGN, NMC, College Paramedics, HCPC)
- Willingness to undertake independent prescribing qualification if not already qualified
- Be up to date with all mandatory training
- Experience of delivering patient care in an urgent care primary care or community setting
- Experience of multi-disciplinary working
- Minimum of 2 years post registration experience
Desirable
- Evidence of continuing post registration development
- Independent prescriber
- Willingness to undertake additional training as agreed with organisation
- Good experience of delivering patient care in an urgent care primary care or community setting
